Position Description:
Provide appropriate intervention services to patients and/or clients as well as their families receiving services within the
Community Living Assistance and Support Services Program. Work typically involves a variety of bio-psychosocial issues, assessment of the social needs as related to the patient's/client's health status, and the development of a plan of services, including advocacy, discharge planning, referrals and practical assistance necessary to accomplish desired objectives. Provides consultation, orientation and training to health care staff on the effects of bio-psycho-social, cultural
and economic issues on the management of patient/client health needs. They also serve as a resource referral service in advising clients and their families about available community resources.

Major/Essential Functions:
+ Participates in Departmental Performance Improvement which will satisfy both institutional and contractual requirements.
+ Employee is responsible for convening service planning team meetings and completing annual renewal and developing IPC.
+ Employee is responsible for completing face to face quarterly review. Responsible for communicating with Direct Service Agency and Individual/Family.
+ Maintain current information on services and products needed by persons with disabilities and be available to conduct presentations as needed. Provides phone coverage.
+ Collaborate with collateral agencies and providers to ensure compliance with Individual Service Plan. Conduct home visits as needed. Be on call on a rotating basis and counsel with clients and provide crisis intervention and advocate as needed.
+ Obtain all necessary authorization to provide services. Perform CLASS billing monthly.
+ Assist with planning, coordination and follow up on WTCMP special projects and provide.
+ CLASS training to new employees. Performs all other related duties, within the scope of practice, on an as needed basis or as required by Supervisor.
+ Adhere to all TTUHSCEP policies, procedures and processes
+ Personally demonstrate, display, and act in accordance with TTUHSCEP’s values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork).
+ Actively promote and encourage staff across the institution.

Preferred Qualifications:
+ Knowledge of social work process, ability to assess client’s needs, ability to compile/maintain information on community resources.
+ Ability to develop positive relationships with clients and collateral agencies.
+ Problem solving and decision making skills.
+ Ability to conduct a client interview and ability to use a personal computer.
+ Previous experience working in an academic medical or health sciences center.
+ Bilingual in Spanish/English.

Pay Statement:
