Position Purpose:
+ The Hospice Intake Coordinator collaborates with hospital discharge planners/social workers, physicians, billing specialists, patients, families, and others to receive information which facilitates admission of Hospice patients; documents appropriate demographic and clinical information for the processing of incoming referrals; coordinates communication process betweenreferral sources, third party payors, and professional/clerical staff; and coordinates referral process from initiation to completion including all follow-up necessary to accomplish expected outcome.

+ Relationship-based Care: Creates a caring and healing environment that keeps the patient and family at the center of care throughout their experience at Mount Carmel following the principles of our interdisciplinary care delivery system.
+ Accurately collects information regarding incoming patient referrals including prognosis, caregiver availability, place of residence, primary physician, continuity of care information including Physician discharge summary, progress notes, results of diagnostic tests and procedures from hospital
+ Prepares patient/family for admission by listing information needed upon admission, and referring them to Hospice Admission telephone line.
+ Researches patient financial information in the insurance verification system in order to expedite patient informed benefit of election (BOE).
+ Researches physician licensing in federal website to ensure compliance regarding Medicare exclusions or any license limitations
+ Communicates Medicaid case numbers with hospice social worker and business operations team. Reports all Medicaid patients through the Ohio Medicaid IVR/Portal system (MITS).
+ Notifies physician and discharge planner of non-admitted patient. Maintaining the electronic data base of admitted, non-admitted patients, and all referrals obtained. Report referrals to management on a consistent basis.
+ Verifies insurance coverage for patient referrals and communicates necessary information to the insurance company, Hospice staff, physician, and independent contractors in timely manner.
Minimum Qualifications:
+ High School diploma/GED
+ Experience: Minimum of one year recent Hospice experience -- or related setting such as home health, hospital, ambulatory care center, physician office in which assessment and critical decision-making skills are required
+ Effective Communication Skills
+ Demonstrated ability regarding critical thinking skills, telephone and direct assessment of patient, responding to emergency referrals, establishing priorities re: scheduling admissions of Hospice patients in all settings-home, hospital, nursing home, and assisted living center.
+ Must be able to work a flexible schedule; may be required to work evenings, weekends, holidays, and on-call.
+ Effective Interpersonal skills, demonstrates tact, judgment and diplomatic business-like manner in dealing with physicians, other healthcare professionals and coworkers.
+ Working knowledge of personal computer and windows-based software applications including accessing and sorting appropriate data repository information related to Hospice referral.
+ Knowledge of reimbursement sources for health care (Medicare, Medicaid, other third party payment sources).
+ Knowledge of Hospice settings of care--understands the durable medical equipment necessary to meet specific needs of population served.
