Description:
We are seeking a dedicated and knowledgeable Asthma Education Specialist to join our team on a per diem basis. This role involves delivering home-based asthma education to patients under the age of 18 with uncontrolled asthma. Working collaboratively with a Community Health Worker and the Asthma Program Coordinator, the Asthma Education Specialist will play a critical role in improving patient outcomes in the Greater Waterbury, Greater Torrington, and Naugatuck Valley regions.
Key Responsibilities:
+ Provide comprehensive asthma education and self-management training to pediatric patients and their families, in accordance with National Heart, Lung, and Blood Institute (NHLBI) asthma guidelines.
+ Conduct home visits to assess environmental triggers and provide tailored recommendations to reduce exposure.
+ Collaborate with a Community Health Worker and Asthma Program Coordinator to develop and implement individualized asthma action plans.
+ Educate patients and caregivers on proper medication use, including inhaler techniques and adherence to prescribed therapies.
+ Document patient progress, interventions, and outcomes in accordance with organizational and regulatory standards.
+ Participate in ongoing training and professional development to remain current with best practices in asthma care and management.
Qualifications:
+ **Education:** Registered Nurse (RN), Certified Asthma Educator (AE-C), or Respiratory Therapist (RRT) with a focus on asthma care.
+ **Experience:** Demonstrated experience in asthma education, management, and patient care, preferably in home or community-based settings.
+ **Knowledge:** In-depth understanding of NHLBI asthma guidelines and best practices in asthma self-management.
+ **Skills:** Strong interpersonal and communication skills, cultural competence, and the ability to work effectively with diverse populations.
+ **License/Certification:** Current licensure or certification in relevant fields (e.g., RN, RRT, AE-C).
+ **Transportation:** Reliable transportation to conduct home visits across the service regions.
**Work Schedule:** This is a per diem position with flexible hours, depending on patient needs and scheduling requirements.
