Description
Basic Function/Overall Responsibilities: The Aftercare Caseworker works collaboratively with the multidisciplinary team to facilitate family engagement, develop a unique relationship with the youth through a community focused lens and significantly contribute to the youth’s transition home. The caseworker will need to remain flexible in the hours worked, duration of a specific workday, and geographic destination of meetings, family engagement, trainings, court proceedings, and community-based appointments for the youth in an effort to support the family of origin and advance the youth’s goal of successful transition home. The Aftercare Caseworker will help the Case Planner regularly evaluate and monitor the comprehensive case management plan, contribute to the family assessment service plan, and develop the discharge plan in collaboration with the clinical team, county stakeholders, and family. The Aftercare Caseworker will facilitate productive relationships with educational and vocational systems with the youth and between families, agency personnel and other community-based service providers. Flexible work schedule: extensive travel throughout New York State, non-traditional working hours required, on call as needed.
Principal Assigned Responsibilities:
+ Work collaboratively with the RTA Case Planner to ensure that referring / custodial agencies and parents are kept closely informed of case movement, case planning decisions and significant events in the life of the youth on an on-going basis.
+ Act as the primary point of contact for family engagement while the youth is residing in the RTA facility and then as a primary point of contact for youth once the youth, enters the Trial Discharge portion of his placement term.
+ Coordinate and collaborate with other treatment team members regarding the youth’s re-entry into their community of origin and keep the clinical team apprised of family engagement developments or changes.
+ Create and cultivate opportunities to empower families to have a voice in their child’s treatment and case planning.
+ Facilitate flexible and frequent contact and visitation with family and other natural supports within their family of origin and home communities.
+ Facilitate and assist in getting families / custodians to the RTA facility for family visitation while the youth remain in the facility, assist in transportation of the youth to their home visits, assist in overcoming geographic and system oriented barriers to visitations and permanency and support the youth throughout the term of placement in these measures to transition the youth home or to their identified permanency resource.
+ Assist the Vocational Coordinator and other members of the clinical team in connecting the youth to continued educational objectives as well as in the search, pursuit and attainment of vocational opportunities prior to facility discharge. During the Trial Discharge period the caseworker will help the youth build an independent capability of financial solvency upon transition back to the community of origin.
+ Provide documented insight and case specific information into agency and New York State databases for assessment and record keeping purposes. As well as provide in-put for risk assessment tool utilized for the youth’s return home and their transition back into the community of origin.
+ Maintain required documentation associated with the provision of services in accordance with regulatory and industry standards via the House of the Good Shepherd’s Electronic Health Record and the New York State Office of Children and Families Connections information database.
+ Report any and all law enforcement contact the youth has while in the community at any point during their term of placement or Trial Discharge to the Clinical Manager or RTA Administration on Call.
+ Perform as the main agency liaison with the youth and family for accountability and fidelity toward adherence to the Conditions of Release during the youth’s Trial Discharge; will demonstrate competent proficiency to this objective as evidenced by completing and submitting legal documentation, e.g. affidavit, when necessary to effectuate the determined consequence or remedial actions required as deemed necessary by the custodial county and / or the RTA program administrative decisions.
+ Attend the initial court proceedings related to the disposition and family engagement of the RTA placement term and attend all subsequent court proceedings whenever feasible to support the youth and family and act as advocate to the youth, family, and community’s best interest.
+ Attend agency in-service trainings, workshops and seminars as appropriate or required by the assigned supervisor or program administrator and other associated tasks as deemed appropriate by the assigned supervisor or RTA program administrator.
Qualifications
Education and Experience Requirements:
+ Bachelor's degree, preferably in human services or a related field, required.
+ Minimum of one-year successful human service experience m residential or human services setting, preferably working with youth involved with juvenile justice.
