Key Responsibilities:
+ Caring for vulnerable patients in their home or other community settings to provide health promotion, disease management, care transitions and other services as needed
+ Work alongside emergency medical services, emergency department and primary care physicians in both the clinic and hospital setting to identify patients who could benefits from community paramedic services
+ Work under the license of the medical director utilizing established written protocols in conjunction with the patient's primary care physician and the patients' established care plan
Schedule is typically Monday - Friday 8am - 4pm. End time can vary between 4-6p, depending on referrals. Flex schedule.
Licensure/Certification Qualifications:
+ State licensure/certification as a Community Paramedic is required in the State of MN upon hire
+ CPR Certification
+ Advanced Cardiovascular Life Support (ACLS) certificate
+ Must meet driving requirements and criteria acceptable to Essentia Health's insurer (non-patient transport)
+ State background check must provide clearance to work with vulnerable adults
+ Current (valid) US/State driver's License
