    Summary:

    The Patient Navigator is responsible for providing health education targeting patients' needs and health risk factors, assisting patients to establish health goals, monitoring patient health outcomes, and serving as a key referral source for patients diagnosed with cancer or identified at high risk for cancer. The Patient Navigator is responsible for helping patients and their families navigate the healthcare system and community-based services by eliminating barriers to care. This is a grant funded position. The Patient Navigator will ensure work aligns with and meets all grant guidelines and deliverables.

    Responsibilities:

    + Provide health resources and education to referred patients focusing on health risk factors.

    + Partner with patients and their families to help navigate the healthcare system and community-based services that target their health and social needs and/or eliminate barriers to care.

    + Serve as a patient advocate and liaison.

    + Assist patients with referrals to community resources such as financial assistance, legal aid, housing, transportation, etc.as appropriate.

    + Assist identified patients with access to medical services and appointments.

    + Advocate for patients to facilitate communications with health care support services or social services.

    + Collaborate with other professionals to assess client needs.

    + Provide financial counseling and education to patients who identify cost as a barrier to care.

    + Continuously expand knowledge and understanding of community resources, services and programs within the community.

    + Build partnerships with healthcare and community resource providers.

    + Build patients understanding and skills to effectively communicate with healthcare providers and organizations.

    + Demonstrates ability to build and maintain strong working relationships with community partners.

    + Participate in community events promoting prevention and education programs.

    + Participate in the development and implementation of Northern Light Health Community Health Improvement Plans.

    + Maintains a safe environment which includes knowing and understanding hospital and departmental policies and procedures.

    + Report and directly address identified environmental hazards when appropriate.

    + Report and directly address violations of patient safety policy and/or protocol when involved or observed.

    Education
