The University of Miami Health System, "UHealth", Department of Pediatrics- Perinatal Care has an exciting opportunity for a Full-Time Case Worker 2 to work in Miami.
The Case Worker 2 monitors outcomes, data collection, and record keeping, and coordinates services for the respective population, including providing information about professional development opportunities, quality improvement strategies, and counseling to improve the client’s social and psychological conditions. The Case Worker 2 (H) assists those experiencing difficult situations by helping them find the community resources/services that they need contributes to the development of family support plans and monitors its implementation and modification.
Core Responsibilities:
+ Conducts situational analysis, which may be at home and/or office visits, mentors, and provides technical assistance as needed.
+ Collaborates with the service agencies and program leadership to provide and administer services appropriately.
+ Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
+ Maintains and reviews records and files to ensure compliance with project guidelines.
+ Collaborates with all funding sources to ensure the availability of adequate resources.
+ Maintains current knowledge of services available and changes applicable rules and regulations.
+ Attends meetings and training, which may include the Early Learning Coalition, United Way, and the Quality Counts Career Center to enhance developmental potential.
+ Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions:
General Description (Purpose and Function):
This direct service position is responsible for providing maternal child health services to Healthy Start clients (pregnant women and infants) participating in the Healthy Start Program. Duties also include service data quality assurance and quality improvement activities. Must be willing to conduct regular home visits with personal vehicle to provide services to clients and interact with other community providers. Position is salaried, and requires working evenings and weekends as needed to ensure that job duties are met.
+ Provide maternal child health services to Healthy Start clients, including: client assessments, comprehensive care coordination and Healthy Start wraparound services including parenting education, breastfeeding support, tobacco cessation, inter-conception care education, childbirth education, and other psychosocial referral services as needed. Work in conjunction with supervisor and multidisciplinary service team to ensure implementation of coordinated services as related to program participant needs.
+ Provide case management/care coordination referrals to participants when appropriate for the range of maternal child health psychosocial and behavioral health needs.
+ Perform quality assurance tasks, service-related administrative duties and data tasks related to the preparation of programmatic documentation and agency reports and compliance with program performance measures.
+ Function as a contributing member of the team during monthly staff meetings and supervision and with other team members.
+ Conduct Healthy Start educational outreach activities in community settings and facilitate collaborative relationships with community partners.
+ Complete all required client-related service and programmatic written and electronic documentation in compliance with programmatic requirements, agency reporting, and the Healthy Start Standards and Guidelines.
+ Attend all programmatically required trainings and staff meetings and strictly adhere to program protocols.
+ Other organizational/support duties as assigned.

CORE QUALIFICATIONS:
+ Bachelor’s Degree in relevant field required
+ Minimum 3 years of relevant experience
+ Skill in completing assignments accurately and with attention to detail.
+ Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
+ Ability to process and handle confidential information with discretion.
+ Ability to work evenings, nights, and weekends as necessary.
