Job Summary:
Position Summary:
Upstate University Medical Associates at Syracuse Inc. (UUMAS) is seeking a Registered Nurse (RN), or Certified Nurse Specialist (CNS), to coordinate and conduct Medicare Annual Wellness Visits (AWVs) across seven Primary Care clinics. This role is essential in enhancing patient outcomes through proactive preventive care management under the supervision of a licensed provider. The RN/CNS will report to the UUMAS Director of Quality and Practice Operations and collaborate closely with Primary Care clinic Medical Directors and Nurse Leadership.
Duties/Responsibilities:
Identify Medicare patients overdue for their Medicare Annual Wellness Visit using established EMR reports.
Schedule and conduct Medicare Annual Wellness Visits in accordance with CMS guidelines.
Perform comprehensive health risk assessments, identify preventive care needs, and document findings in the electronic medical record (EMR).
Collaborate with primary care providers to develop individualized preventive care plans based on patient history and risk factors.
Educate patients on preventive health measures, chronic disease management, and available wellness resources.
Ensure accurate coding and documentation to support quality initiatives and Medicare compliance.
Work closely with clinical teams across seven Primary Care clinics to streamline the AWV process.
Function independently to complete assigned tasks without direct supervision.
Rotate between different Primary Care clinics as required.
Maintain confidentiality and adhere to all HIPAA and institutional policies.
Perform other duties as assigned.
Minimum Qualifications:
Bachelors degree or higher preferred (Associate degree considered with 4+ years of Primary Care experience).
Active NYS RN license required.
Current BLS certification (American Heart Association or American Red Cross) required.
Experience with electronic medical records (EMR) required.
Knowledge of medical office procedures and standards of medical care.
Ability to work independently is a must.
Preferred Qualifications:
Epic experience preferred. 3-5 years of direct experience in a Primary Care setting preferred. Certified Nurse Specialist (CNS) preferred.
Work Days:
Monday - Friday 8:00 am - 4:30 pm

L3: $65,558 - $104,893
Please note the salary information shown is a general guideline only. Salaries are based upon candidate skills, experience, and qualifications, as well as internal equity, market and business considerations. The salary range depicted is based on full time, 1.0 FTE and would be prorated accordingly for less than 1.0 FTE.
