    Job Title: Care ManagerJob Description

     

    The Care Manager is responsible for providing care management services to beneficiaries with planned admissions to acute care hospitals, or to those transitioning from case management or disease management programs. This role systematically identifies and addresses fragmented care for patients with acute, real-time needs and tailors interventions and services to fill gaps and produce optimal clinical outcomes cost-effectively.

    Responsibilities

    + Contact patients with upcoming hospital admissions to discuss expectations and assess their condition.

    + Advise patients on probable length of stay and arrange for services at discharge.

    + Work with physicians and hospitals to enforce treatment plans and orders.

    + Ensure patients receive specialty care and tests as ordered.

    + Coordinate health care team services to avoid duplication and conserve benefit dollars.

    + Evaluate the need for and authorize equipment, supplies, and services.

    + Instruct patients and families in proper care and refer patients back to physicians or other health care team members as needed.

    + Conduct hospital visits and confer with physicians to clarify diagnosis, prognosis, and therapies.

    + Document case summaries in Transitional Care Plans and share them appropriately with beneficiaries and providers.

    + Facilitate beneficiary transfers among regions and collaborate with military liaisons to minimize disruption.

    + Assess patients' benefit plan coverage and negotiate cost-effective rates for provider services.

    + Contact patients within 48 hours of discharge to ensure sufficient support for recovery and compliance with medications and follow-up appointments.

    Essential Skills

    + Graduate of Nursing program or BSN Degree preferred.

    + 2 years of experience in relevant field.

    + Must have and maintain a current, valid, and unrestricted Registered Nurse license.

    + Strong computer skills including Microsoft Office Suite.

    + Case management and managed care experience.

    + Proficiency in telehealth and telephonic communication.

    + Acute care and care management expertise.

    Additional Skills & Qualifications

    + Spanish speaking - Bilingual proficiency.

    + Experience in home health, hospice, or hospital settings.

    + Customer service experience.

    + Call center experience.

    Work Environment

    This is a remote work position, requiring a strong Wi-Fi connection and a dedicated workspace with a closed door. The work schedule is Monday to Friday, 11:30 AM to 8 PM EST initially, with the possibility of transitioning to a 4-day work week after demonstrating performance on contract.

    Pay and Benefits

    The pay range for this position is $35.00 - $41.00/hr.
