POSITION SUMMARY:
The Habilitation Coordinator/QIDP is responsible for the direct supervision of Habilitation Counselors and Habilitation Aides within the Day Habilitation program. This role includes the design, implementation, and ongoing monitoring of individualized program plans that promote independence, inclusion, productivity, and personal growth. The Coordinator/QIDP ensures that all supports provided align with person-centered principles and are in full compliance with OPWDD and HCBS Waiver standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
+ Oversees group programming and supervises assigned habilitation staff through regular face-to-face supervision and completion of annual performance evaluations.
+ Participates as a member of the Interdisciplinary Team (IDT) in the development and implementation of person-centered program plans. Coordinates and attends Life Plan meetings and other individual-specific reviews.
+ Provides direction and coordination for assessments, goal development, and service planning in accordance with agency policies and state/federal regulations. Ensures continuity of services with residential providers.
+ Trains and supports habilitation staff in outcome implementation and goal fidelity.
+ Coordinates and facilitates ongoing in-service training in collaboration with clinical and administrative leadership.
+ Leads informal orientation and onboarding efforts for newly hired habilitation staff.
+ Participates in recruitment, interviewing, hiring, evaluation, and discipline of habilitation staff. Provides ongoing guidance and recommendations to promote performance improvement.
+ Supports the intake and screening process for new individuals entering the Day Habilitation program. Assists with group routines and person-specific engagement as needed.
+ Schedules and attends semi-annual and special meetings for people supported, ensuring documentation is accurate and timely.
+ Monitors individual records for completeness, accuracy, and regulatory compliance.
+ Serves as a liaison to external professionals and stakeholders regarding individual service plans.
+ Ensures all incident reporting and management procedures are completed per OPWDD and NYS Justice Center regulations, including documentation, notifications, and investigative follow-up.
+ Performs other related duties as assigned to support program quality and regulatory compliance
QUALIFICATIONS AND REQUIREMENTS
+ Bachelor’s degree in psychology, social work, rehabilitation, education, or a related human services field (Master’s degree preferred). + Minimum of one (1) year of experience supporting individuals with intellectual and developmental disabilities.
+ Strong knowledge of OPWDD regulations including 14 NYCRR Part 635-10 and HCBS waiver service requirements.
+ Strong organizational, documentation, interpersonal, and data analysis skills.
+ Proven ability to collaborate across interdisciplinary teams and manage multiple responsibilities efficiently.
+ Experience with person-centered planning tools and electronic health records (EHR).
+ Proficiency in Microsoft Office Applications required.
+ Must perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance requirements, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completion of annual corporate compliance training . Fully cooperates in all corporate compliance investigations and reviews.
