Job Function: 01. Ensure competency and communication; successful Completion of BLS and ACLS Curriculum, successful completion of 1 month orientation, communicate and document specific pharmacotherapeutic recommendations to appropriate healthcare personnel including physicians, nurses and pharmacy staff, alert and educate staff regarding new and current initiatives in a timely and organized manner, document clinical activities that include, but are not limited to, disease-state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities, use a documentation program that attaches both a clinical significance and an economic value to clinical interventions.
Job Function: 02. Exercise leadership and project management skills; actively participate in departmental, site-specific and corporate committees, conduct an administrative project that will improve the practice of pharmacy within your institution, a measurable component of the administrative project must be presented at the ASHP Midyear Clinical meeting and UHC Poster session, design and implement a research project that will advance the profession of pharmacy and/or patient care. Research project will be presented at Eastern States Conference and prepared as a manuscript acceptable for publication, participation in recruitment efforts of future pharmacy residents.
Job Function: 03. Provide evidence-based, patient-centered medication therapy management among interdisciplinary teams; collaborate with providers to individualize drug therapies promoting safe, effective and economical patient care, intervene as necessary in the medication delivery process to maximize patient outcomes, review patient medical record for correlation between medications and medical problems, polypharmacy, medication regimen, appropriate medication selection, therapeutic duplication, allergies, drug interactions, adverse events and failure to receive therapy, individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination, review patient drug histories to determine which maintenance drugs should be continued, modified and discontinued during the acute illness, clarifies previously effective dosages and dosage regimens, provide consultative service in pharmacotherapy and pharmacokinetics.
Job Function: 04. Manage and improve medication-use process; active participation in the successful completion of a medication use evaluation, create a drug monograph with a focus on safety, efficacy and cost, the monograph will be presented at the Pharmacy and Therapeutics Committee meeting.
Job Function: 05. Support department and organization in quality, safety, and pharmaco-economic initiatives; examine the pharmaco-economic impact of providing care to the patients, identify adverse drug events, provide guidance and measures to assist in their management and prevention, document interventions and adverse events in appropriate reporting systems.
Job Function: 06. Participate in Quality and Process Improvement teams, development of medication utilization evaluations, and institutional QI activities.
Job Function: 07. Perform other related duties as needed; responsible for continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.
Job Function: 08. Staffing; provide support to Pharmacy Operations every other weekend and in times of demonstrated need.
Education/Experience:
+ Residents must have a B.S Pharm or Pharm.D. degree from an accredited college of pharmacy
+ previous experience as a pharmacy technician or intern is desired, but not required,
+ must be licensed or eligible for licensure as a pharmacist in the state of New Jersey. If not licensed the candidate must be licensed within 2 months of starting their residency.
+ Residents should display evidence of above average academic performance and professional society activity.
