    Ascension is seeking a Case Management Director over the Central Region, which includes our new Women's and Infants Tower, our new Brain and Spine Hospital, and the Heart Center, to direct the Case Management and Social Services team. This position will have direct and indirect reports and ensure all areas are meeting compliance standards and ensuring effective care transitions.

    Responsibilities

    Lead and direct case management services, workflow and resources.

     

    + Formulate and implement operational strategies and initiatives in the case management program.

    + Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.

    + Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.

    + Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

    Requirements

    Licensure / Certification / Registration:

    + One or more of the following required:

    + Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.

    + Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.

    + One or more of the following preferred:

    + Case Manager credentialed from the American Case Management Association (ACMA).

    + Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

    Education:

    + High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required.

    + Master's degree preferred.

    Work Experience:

    + 5 years of experience required.

    + 10 years of experience preferred.

    + 2 years of leadership or management experience required.

    + 5 years of leadership or management experience preferred.

    Additional Preferences

    + Strong understanding of:

    + Discharge planning

    + Medicare/Medicaid regulations

    + Utilization review and management

    + Behavioral health and social determinants of health

    + Proven ability to:

    + Lead teams through change

    + Use data to drive decisions and process improvements

    + Collaborate with medical staff, nursing, and executive leadership

    + Navigate complex patient and system-level challenges
