    Job Summary and Qualifications

    **Position Summary** : Performance Improvement Specialist will provide support to providers within the VCP network. This support will include, but not limited to, monitoring of progress towards network goals, quality care gap management, practice transformation to improve process flows, transitions of care for identified populations, and care coordination to drive quality and cost outcomes for the attributed populations.

    Major Responsibilities:

    + Assess assigned practice workflows and processes that are in place to close gaps in care, make outreach to patients to assist with quality gap closure.

    + Work with excel spreadsheets reviewing various EHR’s to assist with gap closure and data submission.

    + Assist assigned practices with managing their populations that can include, but not limited to, specific chronic diseases, high ER utilizers, medication compliance, and other identified areas of focus.

    + Meet with practices both onsite and virtually to engage practice with network and develop a plan to work towards the identified network goals.

    + Ability to present performance and opportunities to practices including physicians.

    + Monitor practice progress on an ongoing basis, which may include analysis of data and identifying opportunities for improvement.

    + Identifies problematic issues, determines reasons and actions taken to resolve, and escalates appropriately

    + Communicates clearly, regularly and effectively with all levels and functions to create an atmosphere of mutual respect, openness and encouragement.

    + Maintains the confidentiality of the meetings attended.

    + Builds rapport and establishes credibility with the physician leadership and members.

    Education and Experience:

    + LPN or BA required

    + Healthcare experience preferred.

    + Knowledge of primary care practice process preferred.

    + Experience with evaluating and monitoring data around quality metrics preferred

    Knowledge, Skills, & Abilities

    + Proficiency with MS Word, Excel, and Power Point required. Experience with SharePoint preferred.

    + Adaptability – Maintaining effectiveness when experiencing major changes in work tasks or the work environment; adjusting effectively to work within new work structures, processes, requirements, or cultures.

    + Customer orientation - Establish and maintain long-term customer relationships, building trust and respect by consistently meeting and exceeding expectations

    + Energy – Consistently maintaining high levels of activity or productivity; operating with vigor, effectiveness, and determination over extended periods of time

    + Managing Conflict – Dealing effectively with others in an antagonistic situation; using appropriate interpersonal styles and methods to reduce tension or conflict between two or more people.

    + Ability to work independently; be resourceful and proactive

    + Clear and confident communicator internally and with members/external partners

    + Confident public speaking abilities

    + Passionate about improving the healthcare system

    + Unyielding attention to detail

    + Ability to work REMOTE if local to the Richmond Metro area.

    + Ability to travel within the Richmond Metro/Tri-Cities region.
