*Qualifications and requirements*

Applicants should have a current state LPN license from an accredited nursing school and can practice as an LPN per the Virginia Department of Health guidelines. Additional requirements include:

* At least one year of clinical direct patient care nursing experience in an acute care hospital required. Previous Nursing/Case Management/Utilization Review/Discharge Planning experience preferred.

*Department/Unit Summary*

This role coordinates patient care from preadmission through discharge to ensure quality outcomes, patient satisfaction, cost efficiency, and compliance with healthcare regulations. Key responsibilities include collaborating with interdisciplinary teams, managing admissions and discharge planning, monitoring hospital stays, conducting medical record audits, identifying documentation improvements, and utilizing community resources. The position also involves preparing data analyses, staying current with regulatory changes, and supporting smooth transitions across the care continuum.
