Case Manager Crisis Residence full-time

Unity House of Troy’s six-bed Crisis Residence has a fulltime case manager positions available for this short-term stay facility that serves participants for up to 28 days as they prepare to transition back to the community.
Responsibilities:
+ Provide crisis intervention and case management services
+ Provide encouragement and positive feedback
+ Assist with implementation of service plans
+ Help develop Wellness Recovery Action Plan and Crisis Plan
+ Provide accompaniment, support and advocacy as needed
+ Assist participants in building life skills
+ Make referrals to local services and other resources to support participant goals
+ Participate in rotating on-call system
+ Promote a culture of inclusion and belonging
Requirements:
High School diploma or GED required and at least 1 yr of experience working with adults living with a mental illness. Valid NYS Driver License. Reliable transportation. Excellent verbal and written communication skills. Computer skills. Ability to work independently and as a team.
