ABOUT US:
DMHAS is a health care agency whose mission is to promote the overall health and wellness of persons with behavioral health needs through an integrated network of holistic, comprehensive, effective, and efficient services and supports that foster dignity, respect, and self-s ufficiency in those we serve.
With three locations throughout Fairfield County, SWCMHS, promotes and administers comprehensive recovery oriented services in the areas of mental health treatment and substance abuse prevention and treatment. These services include inpatient treatment programs and outpatient clinical services, as well as a wide range of additional supportive treatment services.
ABOUT THE COMMUNITY OUREACH AND ENGAGEMENT SERVICE (COES):
The COES is the “front door” for community-based services in Bridgeport. The Unit coordinates streamlined access to the most appropriate and effective services within SWCMHS. COES provides community outreach, triage, diagnostic and evaluation services, homeless outreach services, short-term treatment services, employment services, peer support, and referrals to community providers. COES is the initial point of entry into the SWCMHS, manages admission to all SWCMHS programs and is designed to quickly identify an individual’s needs and refer them to the appropriate level of care either within SWCMHS or in the community.

PURPOSE OF JOB CLASS (NATURE OF WORK)
In a state agency behavioral health program as part of a multi-disciplinary team at one or more locations this class is accountable for independently providing clinical social work services.
OFFICE OF HEALTHCARE ADVOCATE: This class is accountable for serving as a case manager and advocate for clients that have received or need to receive behavioral health care services.
EXAMPLES OF DUTIES
+ Functions as the clinical social worker/clinician for the Intake Unit at GBCMHC
+ Scheduling of clients referred from area hospitals, walk-ins and client self-referrals
+ Collect and assess all written and oral communication received from outside agencies
+ Interface with other agencies and providers
+ Screens for risk issues and raises any concerns pertaining to a referral to COES/DEC-Intake Supervisor
+ Accepts clients based on GBCMHC admission criteria or refers clients to outside agencies for appropriate level of care
+ Elicits and documents any medical co-morbidities and relays information to the admitting clinician and prescriber
+ Performs comprehensive intake evaluations for scheduled intakes including but not limited to psychosocial assessment, risk assessment, C-SSRS Lifetime, functional assessment (DLA-20)
+ Develops Individual Recovery Plans by collaborating with clients and ensuring services and interventions are implemented, as well as performing the initial assessment of walk-ins
+ Maintains weekly list of current brief treatment clients, following up with GBCMHC clinicians to ensure timely disposition of clients
+ Updates brief treatment list on weekly basis with all relevant information
+ Provides outreach and engagement in the community whenever needed
+ Provides crisis intervention in collaboration with other service providers
+ Provides liaison service to clients who are in transition from long term inpatient beds, the Department of Corrections, or the Department of Mental Health and Addiction Services to community living
+ Follows and complies with SWCMHS Fidelity Standards
+ Maintains clients' rights to confidentiality
+ The Incumbent is required to make entries into the Agency's mental record including WITS progress notes, Recovery Plan, as well as DMHAS databases including Waiver program entries
+ Enters billing data in real or near real time for both billing requirements and regulatory or accreditation standards
+ Ability to comply with agency attendance and work rule requirements
+ Ability to perform CPR and first aid and has current knowledge of safe work practices
+ Complies with all relevant regulations, standards, policies and procedures, and guidelines

MINIMUM QUALIFICATIONS
Licensure as a clinical social worker in the State of Connecticut.
PREFERRED QUALIFICATIONS
+ Experience providing community-based (i.e. office based and non-office based) behavioral health services and working with individuals in crisis and presenting with risk issues
+ Experience working with individuals with severe and prolonged mental health issues, substance use disorders and co-occurring disorders and individuals with histories of legal involvement
+ Experience with clinical best practices including Trauma Informed treatments
+ Experience working within a multi-disciplinary clinical team of professional and paraprofessionals.
+ Experience working in a multicultural environment, utilizing skills, abilities and sensitivities to promote and embrace a global cultural environment of diversity and inclusivity
+ Experience providing individual counseling or psychotherapy
+ Experience with electronic health record (i.e. WITS) and using Microsoft Office Software, including Outlook
SPECIAL REQUIREMENTS
+ Incumbents in this class must possess and retain a license to practice clinical social work pursuant to Sec. 20-195 (m) through (r) of the Connecticut General Statutes.
+ Incumbents in this class must possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. § 1320a-7b(f).
+ Incumbents in this class may be required to have knowledge of sign language and communication ability with the deaf and hearing impaired in certain designated positions.
+ Incumbents in this class may be required to have ability in Spanish oral and written communication in certain designated positions.
+ Incumbents in this class may be required to travel.
