    Bachelor Level - Intensive Care Coordinator

    We have an exciting opportunity for a Bachelor Level Intensive Care Coordinator to join our growing children's mental health program at CSA in Somerville. We are looking for a compassionate and culturally sensitive professional who thrives in a diverse and collaborative environment.

    As a Bachelor Level Intensive Care Coordinator, your responsibilities will include:

    + Provide comprehensive care coordination to children and families.

    + Perform initial clinical assessments to identify family and youth's need and goals.

    + Develop, implement and monitor individualized care plans based on family needs and goals.

    + Facilitate care plan team meetings.

    + Act as a liaison with collateral agencies and programs

    + Advocate for families and educate them on self-advocacy

    + Identify and assist youth and families in accessing necessary services

    + Ensure proper documentation, and safety planning

    + Participate in staff meetings, supervision, and training

     

    Schedule: This position offers a hybrid schedule with flexible options. On-call availability during evenings and weekends on a rotating schedule is required to ensure continuous support for our clients.

    Pay Rate:

    + $22.28/hour

    + additional $0.96/hourly compensation for language skills

    Required Skills

    + Excellent communication and organizational skills

    + Basic computer fluency, including Microsoft Office

    + Valid driver's license and reliable, personal vehicle

    Required Experience

    + Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university

    + Minimum of one-year experience working with children with serious emotional disturbance

    + Community-based experience strongly preferred
