    Job Title: Participant EscortDepartment: Clinical Operations Location: 490 East Ridge Road Rochester N.Y.Hours Per Week: 20 hrsSchedule: Monday thru Friday 4 hours shift

     

    SUMMARY: The Participant Escort will meet and assist ElderONE participants at their scheduled appointments.

    RESPONSIBILITIES:

    + Complete activities as listed within the participant care plan as required during each encounter

    + Meets participants at scheduled appointments

    + Assists participants with checking in and checking out for appointments

    + Provides assistance with needs while attending the appointment including (but not limited to): supervision, transfers, dressing and undressing, toileting, assist with positioning

    + Communicates with Appointment Coordination team and Transportation to review any cancellations, no shows, change in appointment times

    REQUIRED QUALIFICATIONS:

    + Reliable transportation to meet participants at scheduled appointment

    + Experience with elderly population preferred

    + Experience with direct patient care preferred

    + HHA, CNA, PCA preferred

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
