    Care Manager

    Job Description

    This is a full-time hybrid Care Manager role located in the Fox Valley area with some remote work flexibility. The Care Manager will be responsible for case management, training, social services, and collaborating with medical professionals to assure high-quality care solutions. Will be supporting members in Appleton and Green Bay area so must be willing to commute within that range. Will spend 50% of their time meeting with members at their homes are attending doctor appointments with them, the other 50% of the time will work remotely completing additional duties.

    Responsibilities

    + Visit clients to ensure they are receiving the services they are eligible for.

    + Perform remote work tasks such as paperwork, communication with medical payers and providers, and other necessary areas.

    + Collaborate with medical professionals to provide high-quality care solutions.

    + Engage in training and social services to support client needs.

    Additional Skills & Qualifications

    + 2 years of Case Management and Elder Care skills

    + Experience in Social Services and Medicine

    + Knowledge of community resources and support services

    + Bachelor's degree in Social Work, Phycology Sociology, or related field

    Work Environment

    The organization provides a hybrid work environment, with 50% of the time working out of the home and the other 50% meeting with patients and their families. It is a Patient Advocacy company focusing on patients due to direct payment by patients, ensuring a great work-life balance.

    Pay and Benefits

    The pay range for this position is $70000.00 - $80000.00/yr.

     

    401K, Mileage Reimbursement and PTO. Currently, they do not offer Health Insurance but can receive that through Populus Group if they are hired on Contract.
