The Palliative Care Advanced Practice Nurse (APN) provides leadership and oversight in the development and growth of the of the palliative care program. Provides direct and indirect clinical support to patients and families experiencing a chronic and/or terminal illness effecting quality of life. Works in the professional role as an expert clinician, consultant, leader, educator, and translator of research. The APN collaborates with an interdisciplinary team, the referring physician, and coordinates the care of patients and families
Qualifications:
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
+ 1 year in management and leadership, preferred
+ 1 year experience working as part of an interdisciplinary team, preferred
+ 1 year experience as an advanced practice nurse, preferred
+ Current unrestricted RN license in state of CO
+ Nurse Practitioner or Clinical Nurse Specialist in state of CO
+ Board Certified as a Nurse Practitioner or Clinical Nurse Specialist
+ Advanced Certified Hospice and Palliative Care Nurse (ACHPN) through National Board for Certified Hospice and Palliative Nurses preferred.
+ Basic Life Support
+ Advanced Cardiac Life Support preferred
+ Prescriptive authority and DEA preferred
PHYSICAL REQUIREMENTS
Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)
