Value-Based Care Pharmacist (Hybrid)
Position Summary:
Consults and participates as part of a multidisciplinary team. Provides personalized care to patients. Plays a crucial role in assessing medication needs and supporting patients and their families through education.
Job Description:
Primary Responsibilities
+ Provides high-quality medication management to patients enrolled in the Primarily Home program.
+ Conducts medication reconciliation and assessments resulting in individualized care plans tailored to each patient’s needs.
+ Acts as a key educator regarding medications to patients and their families, ensuring they have the necessary resources and understanding to manage their medication regimens in the home.
+ Communicates medication recommendations to the Primarily Home team and/or the patient’s primary care physician.
+ Prepares for and participates in virtual patient rounds.
+ Collaborates closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
+ Enters data and assessments into the company’s record keeping software.
Qualifications
+ Bachelor's degree or equivalent
+ Licensed pharmacist in the state of Illinois and eligibility for licensure in other states
+ 3+ years of relevant experience or the knowledge, skill, or ability to succeed in the role
+ Firm clinical knowledge
+ Good communication, interpersonal, and educating skills
+ Excellent organizational skills
+ Ability to prioritize and work on multiple tasks
Working Conditions
+ While performing the duties of this job, the employee works in normal office working conditions.

The hiring range for this position is:
$113,800.00-$154,525.00
Factors that may be used to determine your actual pay rate include your specific skills, experience, qualifications, location, and comparison to other employees already in this role.
