Job Title: Permanency Specialist (per diem) Department: Foster Care
Position Summary: The Permanency Specialist is responsible for coordinating, facilitating and managing the agency’s Permanency Round Tables held through the Foster Care, Therapeutic Foster Care, Residential and Supervised Independent Living Programs. This position will also ensure all agreed upon follow ups are managed by a variety of participants including Gateway agency staff and collaborative partners.

Compensation
Bachelor's Degree: $21.50 - $23.59 per hour
Master's Degree: $23.30 - $25.38 per hour

Position Specific Duties/Responsibilities:
+ Work collaboratively with casework teams (Gateway team, Department of Social Services, Office of Children and Family Services, School teams, etc.)
+ Run regular meetings regarding “Permanency Roundtables” for youth who do not have a plan to ensure a permanent resource or whose permanency is not certain
+ Complete record reviews of all cases referred for Permanency Roundtables
+ Ensure accurate, timely and comprehensive documentation of all necessary information for the case record
+ Fulfill other duties as assigned
Knowledge, Skills, and Abilities
+ Ability to build rapport with the child/youth’s casework team based on healthy boundaries, effective communication and mutual respect
+ Ability to exercise discretion and judgement in decision making
+ Ability to demonstrate sensitivity, compassion, patience, and active listening
+ Ability to effectively work with individuals from a diverse array of cultural backgrounds, beliefs, customs, and lifestyles
Required Qualifications
+ Bachelor’s Degree in Human Services related field with relevant experience
+ Must have a valid NYS Driver’s License
Preferred Qualifications
+ Master’s Degree in a clinically based area
