    Job Details:

    To render intensive casework, counseling and referral services to all dialysis and pre-dialysis patients and their families. To function as a member of a multi-disciplinary health care team to assist with the transitional and on-going needs of the dialysis patient population. RESPONSIBILITIES

     

    + Functions independently as a counselor within a multi-disciplinary medical model and prioritizes work schedule, assignments and professional decisions accordingly.

    + Determines proper counseling techniques for presenting social pathology of patient and family.

    + Develops and establishes a plan of care after initial assessment of patient.

    + Regularly meets with patients and families in the hemodialysis units, CAPD monthly clinics and home visits to further assess and identify social problems and update patient’s plan of care.

    + Participates in departmental committees; i.e., QA/QI, Dialysis Committee, etc.

    + Maintains appropriate documentation in the medical record, patient care plan, and QA reports.

    + Oversees transient and transfer coordination for the dialysis department.

    + Provides supervision of per diem CSW and is available to staff for crisis intervention as needed.

    + Acts as a liaison between patient and family with other departments; i.e., billing, discharge planning, etc.

    + Will facilitate transplant issues with patients, families and transplant centers and will meet regularly with patients individually and in monthly PD clinics to discuss transplant eligibility and to update patients plan of care.

    + Performs any other duties as assigned.

    QUALIFICATIONS/REQUIREMENTS EXPERIENCE

    + Minimum – 2 yrs. experience; preferred – 4 yrsof experience, 1 yr. Hospital experience

    EDUCATION

    + Minimum – master's degree; preferred – certification in social work

    LICENSES/CERTIFICATIONS

    + NYSDepartment of Education license of social service
