Description
The health services that became theAnthony L Jordan Health Corporation (http://www.jordanhealth.org/) (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.
Jordan Health is now seeking aPPC Program Managerwho, under the guidance of the Associate Chief Medical Officer, is primarily responsible for ensuring the highest quality of care for the patients in the PPC department, while maintaining solvency and aligning with the mission of the organization.

Requirements
ThePPC Program Managerwill ensure:
• The programmatic success of the department
• The financial success of the department
• Clinical excellence in the department
• Active participation and connection of patients to all aspects of Jordan Health.
• To demonstrate positive leadership.
Education And Experience Required:
• Masters’ level training is preferred in areas such as public health, health services administration.
• Equivalent experience will be considered with a Bachelors’ level degree.
• A minimum of one year experience managing grants or programmatic leadership is preferred.
Licenses And Certifications Required:
• Current provider BLS/CPR through the American Heart Association.
• NYS Infection Control Certificate.
• Must possess a drivers’ license.
Special Skills, Knowledge Required:
• General knowledge of ambulatory and community health care.
• Experience with program management, casework supervision, health education and outreach is preferred.
• Must have strong management skills and experience with common personnel and staffing matters.
• Experience working with various ethnic, cultural, racial, gender, sexual orientation and socioeconomic backgrounds is required.

Salary Description
$73,400-$77,510/ANNUALLY
