Pay: Starting at $45 per hour
About the Position
As the Resident Assessment Coordinator, you will be primarily responsible for overseeing the complete processing of the Minimum Data Set (MDS) and the coordination of the Restorative Nursing Program (RNP). The MDS includes initial assessment, periodic updates, coordination of clinical team’s input, presentation of data, and the development of the care plan and follow-up reports. You would be primarily responsible for the development, implementation and monitoring of the Restorative Nursing Program for residents to improve and maintain functional ADL's for residents.
Key Duties
+ Coordinates nursing data input on MDS and develops care plan according to departmental procedures.
+ Coordinates the Resident Assessment Instrument (RAI) process. Opens, directs the timely completion, and locks the MDS ensuring compliance with all federal and state regulations related to MDS assessments.
+ Oversees, initiates, and ensures completion of MDS documentation including but not limited to MDS Charting, IDT assessments, accurate Diagnoses with appropriate ICD-10 codes for admissions, re-admissions, significant changes, annual and quarterly reviews.
+ Ensures that the MDS/RAI deadlines and accuracy requirements are met while ensuring that coding most appropriately captures each patient's acuity and needs for accurate and optimal reimbursement.
Job Requirements
+ Associate’s degree required; Bachelor’s Degree preferred.
+ Current RN license in good standing in VA.
+ CPR certification.
+ Extensive MDS 3.0 experience needed. 3+ years as an MDS/PPS Coordinator.
+ Minimum of 5 years experience in Gerontological Nursing required, preferably obtained in a long-term facility.
+ RAC-CT certification required within one year of employment if not certified at the time of hire. Must maintain certification as current once certified.
