    Summary:

    + The Licensed Practical Nurse maintains accountability for the delivery of safe and competent nursing care to assigned patients. Provides care by using the nursing process under the direction and supervision of a Registered Nurse. Supports and assists the Registered Nurse in provision of care and collaborates with physicians and multidisciplinary team members to achieve the highest standards of care specificity to the age of the patient.

    Responsibilities:

    1. **Utilizes and records the nursing process as delegated by the Registered Nurse.**

     

    1.

     

    1. Collects and documents selected data.

    2. Identifies patients’ needs.

    3. Evaluates health related data, including psychosocial information, and participates in identifying solutions.

    4. Collects data in a systematic ongoing process.

    5. Answers patient call lights promptly

    6. Accurately maintains patients’ charts

    7. Detects changes in patient’s condition

    8. Measures and records patient vital signs

     

    1.

     

    1. Admission process and ongoing assessments are based upon collection and interpretation of data relevant to health status of patient. Appropriate forms are completed. Patient problems are identified and supported by assessment data. Patient assessments are documented within 8 hours of admission and early in the shift. Assists with planning patient care individually taking into account his/her limitations and/or specific requirements.

    1. Patient calls are answered promptly

    2. Timely changes are made to the plan of care as needed

    3. Completes medical forms, reports, evaluations, studies, charting, etc. in timely manner

    2. **Participates in the promotion, maintenance, and restoration of health, provides comfort measures for a dignified death.**

    1. Practices clinically competent and caring behavior

    2. Assists with preparation of plan of care with patient/family/significant other.

    3. Assists in identifying changes and trends in patient’s status.

    4. Participates in modifying nursing interventions based on changes in patient’s status.

    5. Administers prescribed medications or intravenous fluids

    6. Collects samples such as stool, urine and sputum from patients

    7. Administers treatments and special procedures as directed by Registered Nurse or Physician

    8. Responds to patient requests for comfort measures promptly. Nursing care is provided by incorporating the principles of pain management, medication administration, nutrition, skin and airway integrity, elimination and mobility. Problems and changes in patient’s condition are reported to Charge Nurse/MD as they occur. Interventions and patient responses are accurately documented and reported throughout the shift. Patient is able to verbalize plan of care. All orders are completed for the shift.

    9. Completes medication and treatment passes in a timely manner and in accordance with hospital policy.

    10. Specimens are collected according to Standards

    3. **Implements care that is legal, ethical and respectful of patient’s rights.**

    1. Respects and maintains patient’s right to privacy and confidentiality.

    2. Seeks assistance of Registered Nurses and available resources to help formulate ethical decisions.

    3. Delivers care in a manner that preserves and protects patient autonomy, dignity and rights.

    4. Expresses sensitivity to patient’s culture/religious diversity.

    5. Patient information is communicated only to those identified as aiding in care. Patients are informed of their rights and their questions are answered. Charge Nurse is informed during shift of ethical concerns patient expresses, and guidelines are followed as instructed. Care is provided to each patient in a nonjudgmental and nondiscriminatory manner.

    6. Maintains confidentiality of all patients information

    7. Ensures that all nursing care is provided in privacy

    8. Reviews complaints and grievances made by patients

    4. **Provides a safe, secure patient environment.**

    1. Follows Safety and Infection Control Principles.

    2. Completes treatments, medications, and IV administration following defined policies and procedures.

    3. Provides patient comfort and hygiene measures.

    4. Participates in plan of care based on age/population specifications.

    5. Universal precautions and isolation procedures are followed. Hazardous waste is properly disposed. Safety precautions are implemented. Medication and IV calculations are accurate. Nursing care is provided in accordance to age and corresponding stages of growth and development.

    6. Follows proper hygiene and comfort procedures for patients and their environment

    5. **Collaborates with other health care team members in providing care.**

    1. Maintains accountability for duties accepted, including care given by self and by others to whom care is delegated.

    2. Seeks and utilizes appropriate referral processes.

    3. Participates in planning and implementing a multidisciplinary approach to providing care.

    4. Provides leadership to unit support nursing staff

    5. Patients’ follow-up care process is identified prior to discharge. Plan of care is developed and implemented with aid of case managers and other members of health care team. Assignments accepted from the Registered Nurse are completed utilizing established policies, procedures and practices. Duties delegated to nursing assistants are within their scope of practice.

    6. Provides and receives report upon beginning and ending shift.

    7. Supervises the Health Care Partner’s delivery of care and proper implementation of the standard of care.

    8. Performs administrative duties in a timely manner.

    6. **Acquires and maintains current knowledge in nursing practice.**

     

    1.

     

    1. Seeks knowledge and skills needed for the practice setting.

    2. Demonstrates accountability for/promotes own growth and development.

    3. Participates in unit/house wide continuing Quality Improvement activities.

    4. Ongoing educational activities related to clinical knowledge and professional issues are maintained yearly. Maintains current licensure to practice. Participates in house wide and/or unit teams or committee task forces. House wide/unit continuing education offerings are maintained and current.

    5. Maintains current knowledge and applies new knowledge and skills

    7. **Demonstrates a positive attitude and a courteous, professional image at all times.**

    1. Communication with co-workers and manager are harmonious. Patients’ surveys will reflect satisfaction with care received.

    8. **Performs duties with attitude in accordance with hospital policies and philosophy.**

    1. Ensures unit/hospital cost containment contribution through efficient use of time and resources. Utilizes appropriate Safe Patient-Handling equipment, where indicated, to ensure personal patient safety.

    + Graduation from an approved School of Licensed Practical Nursing.

    + **LICENSURE/REGISTRATION/CERTIFICATION**

    + Current licensure to practice as a Licensed Practical Nurse in the state of North Carolina. BCLS Certification.
