    We’re seeking a **Physical Therapist (PT)** to provide skilled home health visits, assessments, and admissions to adult clients throughout **Sussex County** , including **Georgetown, Millsboro, and Milford** .     What You’ll Do:

    + Visit clients in their homes to assess mobility, balance, and safety

    + Develop and implement individualized care plans and evidence-based interventions

    + Provide education to clients and families to support independence

    + Collaborate with a multidisciplinary care team

    + Document care electronically using a state-of-the-art tablet

    What You’ll Need:

    + Active **Delaware PT license**

    + Graduation from an accredited PT program

    + Ability to work independently, with strong communication and time management skills
