POSITION PURPOSE
The Program Lead will collaboratively develop and oversee marketing initiatives, community engagement efforts, establish collaborative strategies with local organizations to create resources aimed at empowering the population and reducing hospitalization rates in collaboration with Nursing, Case Mgt, Social Work and Community Health teams. This position focuses on the Caregiver initiative program development and evaluation, financial oversight and business model development for existing and proposed department programs, grant writing, public speaking and relationship building. This position connects patients and community members to the Caregiver Initiative, and community resources.
_This position is full-time, for a total of five days a week, which may include a combination of weekend days and/or evenings._
*This position is a temporary grant funded position and is only sustained based upon approved grant funding which runs through December 31, 2025. Further grant funding for the position after December 31, 2025 is being pursued however grant funding is required for continuance of the temporary role. Should funding not be available, the position will be discontinued without Severance at 30 days notice.*
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Education:
+ Master's degree in a behavioral science, or related field and 3 years of relevant work experience in a clinical setting or the equivalent combination of education, training and experience where the required knowledge, skills and abilities have been acquired.
+ LMSW.
+ BSN
Experience:
+ 3-5 years current experience in the area of case mgt, discharge planning, community resource utilization
Licensure / Certification
+ Current licensure to practice applicable clinical field in the state of Michigan.
Other Required Skills:
+ Knowledge of MS office/Internet applications.
+ Strong interpersonal and organizational skills. Excellent written and oral communication skills.
+ Demonstrates initiative, resourcefulness, and problem-solving abilities.
+ Project management skills-work plan, meet deliverables, reporting, logistics, communication and budget.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Job Responsibilities:
+ Develops mechanisms for budget and grant management.
+ Tracks grant requirements and progress toward outlined Caregiver Initiative goals.
+ Develops and tracks evidence to measure and report on "return on investment", charity care and other potential deliverables.
+ Develops strategies for the Caregiver program to maintain loyalty among members, patients, and families as well as attract new members and patients.
+ Promotes development of Caregiver educational programs / activities in order to provide outstanding clinical services to maximize impact, external recognition and growth in coordination with other departments.
+ Gathers, organizes and analyzes data to assure creation of reports and statistics.
+ Leads/Participates in quality improvement initiatives and outcome measures.
+ Creates program evaluation measures and customer satisfaction activities.
+ Participates in marketing, communication, strategic planning, policy development with other team members and director.

+ Organizational skills necessary to manage large and complex projects.
+ Knowledge of analytical tools and methods to measure and evaluate productivity, outcomes and improvement methodology in a clinical setting.
+ Knowledge of federal, state, and local regulations, related to utilization management and quality management in healthcare.
+ Excellent customer service skills to deal effectively with various levels of hospital personnel, outside customers and community groups.
+ Ability to lead cross functional teams, gather and exchange information with both internal and external individuals.
