    Job Summary and Qualifications

     

    The Clinical Pharmacist is responsible for providing general clinical and operational pharmacy services. This includes supervision of technical staff.

    What will you do in this role:

    + Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear or potentially unsafe or inappropriate

    + Interprets medication orders (verbal and written), transcribes to computerized patient medication profiles accurately, follows facility practices and procedures for documentation in the patient’s medical record and trouble shoots/clarifies Centralized Order Entry (COE) orders as appropriate

    + Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines.

    + Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-lab test interactions, allergies and appropriateness of drug and dose, indications and therapeutic duplications

    + Suggests appropriate, cost-effective therapeutic medication alternatives to medical staff

    + Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection

    + Provides accurate and timely medication information to healthcare staff and patients and performs applicable medication reconciliation

    + Assists with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and A21 programs as defined by Pharmacy Leadership

    + Participates in rounds and/or direct follow up with providers. This may include interdisciplinary rounds, rounds with individual providers, onsite/in person follow up with providers, and via phone or written communication based on urgency/ability to meet with provider in person

    + Assists with the antimicrobial management and other clinical programs

    + Documents all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions A21

    + Supports and participates in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, HCAHPS and Leapfrog medication related initiatives

    + Completes notification reports regarding medication errors and adverse drug reactions

    + Participates in the training of new pharmacists, pharmacy students, and technicians

    + Models, mentors, and encourages pharmacy students and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity

    + Maintains current pharmacist licensure, and maintains competence required for current job title/position. Pursues board certification or other training/certific+A12ation for clinical area of expertise based on job role

    + Has knowledge of the patient population served and applies that knowledge in the care of those patients

    + Provides basic technical support for automated dispensing machines and other pharmacy computer systems

    + Effectively supervises pharmacy operation activities and provides support to the whole pharmacy team

    + Compliant with facility pharmacy schedule requirements to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team.

    + Effectively and proactively intervenes in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions

    + Trains and participates with Disaster Team drills and real-event response as assigned

    + Trains and participates with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate.

    What qualifications you will need:

    + Pharmacist

    + Bachelors Degree, or Doctorate Degree, or Pharmacy Practice Residency - PGY1

    + Current pharmacist license in good standing with the appropriate State Board of Pharmacy; Board Certification preferred

    + 5 years hospital experience preferred. 3 years HCA inpatient hospital preferred
