    Hours: Tuesday to Saturday or Sunday to Thursday 930 am to 6 pm

     

    Salary: $87,500 LMSW/LCSW/LMHC/LMFT or LP-MHC/MFT $80,000

    LMSW/LMHC/LCSW/LMFT Required

    Working as an CCBHC Outreach Therapist allows you to make a significant impact on the lives of individuals who may not have the current ability to access traditional mental health services. By providing therapy and support outside of the traditional clinical settings, you contribute to breaking down barriers and promoting mental well-being within the communities we serve.

    PRIMARY FUNCTIONS :

    + Client Assessments: Conduct comprehensive assessments to gather information about clients' mental health history, current concerns, strengths, and needs, and identify any potential risks or safety concerns.

    + Treatment Planning: Collaborate with clients to develop individualized treatment plans that address their specific goals, utilizing appropriate therapeutic modalities and interventions.

    + Therapy Sessions: Provide individual sessions using evidence-based approaches such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), or solution-focused brief therapy (SFBT).

    + Crisis Intervention: Offer immediate support and intervention to individuals experiencing mental health crises, utilizing crisis management techniques and connecting them to appropriate resources when necessary.

    + Psychoeducation: Educate clients and their families about mental health conditions, coping strategies, and available community resources to enhance their understanding and promote self-advocacy.

    + Outreach Activities: Develop and implement outreach initiatives to engage individuals and communities who may face barriers to accessing mental health services.

    + Collaboration: Work collaboratively with multidisciplinary teams, including psychiatrists, psychologists, social workers, and other healthcare providers, to ensure coordinated care and holistic treatment approaches for clients.

    + Documentation: Maintain accurate and confidential client records, progress notes, and treatment plans, adhering to all legal and ethical guidelines.

    + Professional Development: Engage in ongoing training, supervision, and continuing education opportunities to stay current with therapeutic techniques, research, and best practices.

    + Program Evaluation: Contribute to the evaluation of outreach programs, attend meetings, and provide insight and recommendations for improving the quality and effectiveness of services.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

    + Assignments may be determined at a future date

    QUALIFICATIONS AND ATTRIBUTES :

    + Experience providing therapy to diverse populations, including individuals from different cultural backgrounds, ages, and socioeconomic statuses.

    + Knowledge of evidence-based therapeutic approaches and interventions.

    + Strong assessment and diagnostic skills.

    + Excellent interpersonal and communication skills.

    + Ability to establish rapport with clients and create a safe, non-judgmental therapeutic environment.

    + Familiarity with documentation and record-keeping procedures.

    + Strong organizational and time-management skills.

    + Ability to work independently and as part of a team.

    + Pass required background checks and clearances.

    + Strong organization skills with accurate attention to detail

    + Excellent communication skills and the ability to multi- task

    + Hands-on experience with network administration.

    + Able to adapt and work in a fast-paced environment

    + Sufficient skills to communicate effectively with system users regarding technical and business issues.

    EDUCATION AND EXPERIENCE :

    + Master's degree in counseling, social work, or a related field.

    + NY Licensure as a therapist (e.g., Licensed Master Social Worker, Licensed Clinical Social Worker, Licensed Mental Health Counselor)

    + Driver’s License
