    Responsibilities:

    + Acts as the first point of contact for incoming requests and supports them in completing intake forms and documents.

    + Responsible for coordinating the intake, scheduling and care delivery process for people with developmental disabilities.

    + Conduct interviews with people to compile intake information.

    + Make referrals to service providers based on people’s needs and assessment.

    Qualifications:

    + Bachelor’s degree in health and/or Human Services PLUS (1) yr experience working with people w/ developmental disabilities OR (1) yr experience as service coordinator/case manager OR

    + Associate’s degree in health and/or Human Services PLUS (3) yrs experience working with people w/ developmental disabilities OR (3) yrs experience as service coordinator/case manager OR

    + High school diploma PLUS (6) yrs experience working with people w/ developmental disabilities OR (6) yrs experience as service coordinator/case manager

    Position Summary:

    Assists persons with disabilities, their families, caregivers, advocates and providers to make informed choices regarding determining and accessing necessary and available supports and services. Performs person-centered planning related to linkage and referrals to access services, monitoring and follow up, advocacy and record keeping. Creates connections with community partners (i.e. schools, vendor fairs, etc.) to enhance The Arc Erie County New York experience. Promotes the agency in a positive light as they are the first point of contact for The Arc Erie County New York.

    Reporting Relationship:

    Receives direct and on‑going administrative supervision from the Coordinator of Central Intake & Admissions.

    Job Duties and Responsibilities:

    + Assess individual’s entitlements and benefits, ensures eligibility for Medicaid and Home and Community Based (HCBS) Waiver Services (if applicable) is maintained. Ensures all necessary documentation is completed to avoid interruption of services & benefits.

    + Ensures the timely completion of initial Level of Care Determinations (LCED’s) for all potential HCBS waiver enrolled individuals.

    + Follows all policies and procedures with regards to Intake, Admissions, Discharges, etc.

    + Maintains data related to admissions, discharges, change of vendor, referrals, denials, waitlists, etc.

    + Advocates by actively supporting, encouraging, and or negotiating for particular programming purposes or outcomes on behalf of and together with the person.

    + Develops and maintain a thorough working knowledge of available services and supports (both traditional OPWDD/OMH/County funded and community based resources).

    + Convenes program tours and intake meetings with individual and circle of support.

    + Maintains awareness of generic and specialized services and supports available to persons with disabilities. Maintains knowledge of service continuum and how to access it.

    + Evaluates service eligibility and secures appropriate documentation.

    + Links and refers for needed services, monitors and follows up.

    + Provides outreach to schools, community organizations and other agencies.

    + Negotiates admissions, service approvals, start dates with various departments, OPWDD and circle of support.

    + Meets with program, personnel to determine program availability, waitlists and program changes.

    + Upload and complete enrollment/termination documents via CHOICES/MAPP platforms

    + Other duties as assigned based on needs of the department.

    Qualifications and Education Requirements:

    Bachelor’s Degree in Health and/or Human Services PLUS one year of experience working with people with developmental disabilities OR one year of experience as a service coordinator/case manager with any population.

    OR

    Associate’s Degree in Human Services PLUS three years of experience working with people with developmental disabilities OR three years of experience as a service coordinator/case manager with any population.

    OR

    High School Diploma PLUS six years experience working with people with developmental disabilities OR six years experience as a service coordinator/case manager with any population.

     

    *Acceptable degrees include: Social Work, Sociology, Psychology, Health, Nursing, Medicine, Rehab Counseling, Therapeutic Recreation, Nutrition, Occupational Therapy, Physical Therapy, Speech Pathology, Audiology, Music Therapy, Education or Special Education.

    + Mental Requirements:

     

    The ability to read, write, analyze, be perceptive and comprehensive, use professional judgment to access needs of individuals; have good math & decision making skills; ability to maintain required documentation in an organized manner, independent action, planning & prioritizing and follow directions. Work independently as well as cooperatively with others. Must have good visual, oral and written communication skills to observe and assess needs and maintain documentation of services provided.

     

    + Human Relations Skills:

     

    The ability to communicate effectively with individuals, families, advocates, & providers in a positive manner, demonstrate the ability to be a leader and serve as an appropriate role model. Receive supervision instruction in a positive manner. Adhere to and administer applicable federal, state, county and agency procedures, rules and regulations. Maintain a professional attitude and appearance at all times.

     

    + Equipment Operation:

     

    The ability to operate business equipment: telephones, fax machines, computer hardware and software, including applications and programming, copiers, calculators, etc.
