Program Support Specialist-Family Bridge

Works with a team of nurses and community health workers (CHWs) to coordinate ECHC Family Bridge (FB) cases, promotes FB to eligible families during or after the hospital stay, serves as FB primary point of contact for the public, and monitors customer satisfaction and referral outcomes. Must be able to interact with and coordinate a large number of supportive services. Must learn the Results Oriented Management Accountability (R.O.M.A.) and Human Services Infrastructure (H.S.I.) Systems. TVCCA operates under these two management principles and all employees must understand and adopt them. This will be a Part-Time Position of 20 to 24 hours per week.
Required Skills
• Bachelor’s Degree plus two years of related work experience, or associate degree plus four years of related work experience.
• Excellent computer skills (Microsoft Office Suite, Excel, Access, and Publisher). Must be able to operate a Windows-based computer and be willing to learn a suite of software systems required for completing reports, grants, and client records.
• Strong written and verbal communication skills.
• Bi-lingual preferred.
• Familiarity with the social determinants of health.
• Have a valid driver’s license, good driving record and access to daily, reliable, insured transportation.
