We are hiring a Hospice Clinical Director/RN Case Manager to join our care team with Hospice Services of Massachusetts in the Wareham area!

The Clinical Director/RN Case Manager is responsible for the overall direction of hospice clinical services. The Clinical Director establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
Experience & Education:
+ Must possess an active and unencumbered RN license in Massachusetts.
+ Bachelor's degree in nursing.
+ Prior experience in hospice required.
Duties & Responsibilities:
+ Oversees all direct and indirect patient services provided by clinical staff.
+ Provides clinical oversight, mentoring, and guidance to interdisciplinary team members.
+ Assists with hiring, evaluation, and supervision of clinical personnel.
+ Develops and maintains clinical policies and procedures that comply with federal and state regulations and CHAP standards.
+ Provides orientation and ongoing training to clinical staff; oversees in-service education.
+ Maintains quality improvement initiatives, monitors performance indicators, and ensures compliance with the performance improvement program.
+ Monitors and documents the patient’s condition, symptoms, and response to interventions.
+ Communicates effectively with families, caregivers, and providers regarding goals of care and clinical updates.
+ Participates in IDT meetings and care conferences.
