    Your Specific Responsibilities:

    This is a clinical services position at the Florida Department of Health in Walton County providing case management and generalized registered nursing support and assistance to providers. This position primarily supports the Primary Care program but may assist in other clinical programs as needed and in accordance with agency policies, procedures, protocols, and guidelines. Will be primarily working out of the Santa Rosa Beach site, but may be required to work at other sites as needed. This position is a key custodian for the pharmacy, laboratory and other assigned areas. The person in this position will be performing tasks involving sensitive information or requiring a high level of trust for other reasons. Adheres to Information Security policies and procdures. Works with co-workers and supervisor to maintain alignment of operations between all CHD sites.

     

    This position will follow Florida Department of Health, Florida Department of Health Walton and Walton Community Health Center policy and procedures.

    This position will provide patient-centered interactions:

    + Respect patient and family values and expressed needs.

    + Encourage patients to expand their role in decision-making, health-related behaviors, and self-management.

    + Communicate with their patients in a culturally appropriate manner, in a language and at a level that the patient understands.

    + Provide self-management support at every visit through goal setting and action planning.

    + Obtain feedback from patients/family about their healthcare experience and use this information for quality improvement.

    This position will assist in the provision of care coordination:

    + Link patients with community resources to facilitate referrals and respond to social service needs.

    + Track and support patients when they obtain services outside the practice.

    + Obtaining test and referral results and communicating with community organizations, health plans, facilities, and specialists.

    + Follow-up with patients within a few days of an emergency room visit or hospital discharge.

    + Communicate test results and care plans to patients/families.

     

    Registered Nursing Tasks

     

    General Care Coordination/ Chronic Disease Case Management

     

    + Will function as a chronic disease case manager and will ensure case management services are meeting the goals of all grant requirements and client needs.

    + Works with providers and clients to provide current high-quality care management services

    + Will track current caseloads and activities in HMS and will document all care and case management activities in the progress notes of HMS.

    + Will work closely with providers and clients to aid clients in completing appropriate medical appointments, referrals, diagnostic tests, and compliance with self-care instructions.

    + Will provide individualized education and support to clients to help those clients manage their health conditions. There will be an overarching goal of improved general health for clients. Education will be client need and learning level specific and appropriate.

    + Will Communicate test results and care plans to clients and or their family.

    + Link clients to community resources to facilitate referrals and respond to social service needs.

    + Will coordinate chronic disease case management data collection with appropriate project manager(s) to assure reporting requirements are met in an accurate and timely manner.

    + Will Track and support clients who obtain services outside their medically centered home Will Follow-up with clients having emergency department visits or hospital discharges.

    + Will have RN telephone triage duties. Will use approved protocols to manage client triage.

    + May provide group education/ classes on specific health topics.

    + Orders and Stocks educational material for clinic client education.

    Other Direct Client Care:

    + Provides health / education counseling directly to clients and their family.

    + Assists Provider with procedures as needed.

    + Administers oral, intramuscular, topical, subcutaneous, and inhalation medications.

    + Administers vaccines according to current ACIP recommendations.

    + Provides nurse visits for things such as blood pressure checks, education, STD treatments, immunizations, Insulin Distribution Program and other services following provider orders and all agency protocols.

    + Conducts/Provides RN visits for Family Planning (FP) which may include Pregnancy Testing, FP Supply appointments and/or STI (Sexually transmitted infection) treatment visits. Will adhere to RN protocols and all FP policies and guidelines.

    + Performing hearing screens and vision screens. Performing EKGs, and other in-house diagnostics. Perform nursing triage and complete workups for provider visits. Provides phlebotomy and specimen collection services including specimen collection, order entry, labeling of specimens and proper storage of specimens prior to shipping.

    + Responsible for efficient clinic flow by working at a speed that maximizes provider productivity and minimizes client wait times.

    + Other assigned duties within scope of practice.

     

    Facility Maintenance

     

    + Cleans exam rooms and direct patient care equipment after each visit and ensures clinic exam rooms are client ready before each visit.

    + Empties trash and disposes of supply boxes and or containers.

    + Secures full sharps containers, stores full sharp containers in designated area, and replaces containers as needed in clinic areas.

    + Cleans spills immediately when observed or discovered.

    + Immediately reports non-functioning or broken equipment to direct supervisor.

    + Assigned purchasing card responsibilities; will adhere to purchasing card policies and procedures.

    Other:

    + Attends and participates in monthly general staff meetings and departmental meetings.

    + Completes all job-related assigned trainings.

    + May participate on teams and/or committees with approval from supervisor.

    + Takes an active role in personal and professional growth.

    + Keeps current on disease processes, and changes in provisions of services.

    + Acts as a positive role model to other DOH staff.

    + Completes Employee Activity Reports (EARS) and submits timesheets according to agency standards.

    + Performs other duties as assigned.

    + Assigned purchasing card responsibilities related to travel. Will adhere to purchasing card policies and procedures.

    + May provide back-up support for Family Planning RN Clinic, Immunization Clinic and/or Epidemiology as needed.

    + Will be assigned the afterhours on-call for client phone calls, abnormal lab result notification and urgent follow ups

    Required Knowledge, Skills, and Abilities:

    Possesses excellent telephone/communication skills. Ability to teach health care relevant to the client’s needs and at the client’s learning level. Ability to communicate clearly and in a manner that promotes client’s confidence, acceptance and understanding of medical information. Ability to establish and maintain effective working relationships with others. Demonstrates: Good judgment, critical thinking skills and telephone “charisma”. Ability to communicate effectively. Ability to assist in planning and conducting a variety of training activities. Knowledge of nursing and public health principles, practices and techniques. Knowledge of human anatomy and physiology. Ability to administer medications. Ability to provide preventative health care and treatment to clients. Ability to use and maintain clinic equipment such as the centrifuge, autoclave, EKG machine, nebulizer, pulse oximeter, manual and digital scales, stadiometer, audiometer, vision screening machine, glucose machine, eye wash station, electronic blood pressure machine, manual blood pressure cuff, and hemoglobin machine. Working knowledge of immunization schedule, use of VIS statements, FL Shots system. Ability to work with patients having a variety of physical and/or mental problems. Ability to clearly document care/services provided. Working knowledge of all DOH- Walton and Walton Community Health Center’s clinical policies, procedures and/or protocols. Knowledge of community resources. Tolerates ambiguity well. Ability to plan, organize and coordinate work assignments. Strong time management skills. Ability to remain flexible and adaptable in the work environment. Experience in crisis intervention and/or teaching coaching. Ability to evaluate situations, develop an action plan and adjust activity and behavior to meet goals and objectives. Ability to learn and utilize a variety of computer programs such as Outlook, Word, Excel, and HMS. Must be able to lift and carry 25 lbs. Must be able to stand and walk for an extended period of time.

    Qualifications:

    Minimum- Licensure as a Professional Registered Nurse , current valid state driver’s license, current CPR/BLS certification.

     

    Preferred – Previous experience in case management, patient education, and primary care, woman’s health, and prenatal experience.
