The Jones-Harrison community offers Assisted Living, Memory Care, Skilled Nursing and Long-Term Care with a beautiful view of Cedar Lake and pristine grounds. This community features 53 Assisted living, 9 memory care apartments, 120 skilled and long term care beds. We are seeking a LPN- Licensed Practical Nurse to support our residents in our Skilled Nursing Facility (Transitional Care) in Minneapolis, MN! This role performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures.

This Nursing schedule includes;

+ 72 hours every two weeks

+ Day shift. Week 1: M, T, W, TH, SA, SU. Week 2: T, TH, F

+ Includes every other weekend Saturday/Sunday and holiday rotations

Responsibilities:

+ Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices

+ Lifts and transfer residents as needed, Initiates emergency treatment as needed

+ Monitors work and performance of unlicensed staff

+ Updates RN with current observation and completes documentation of resident health status

+ Communicates changes in residents conditions to relevant individuals

+ May support RN by completing resident assessments, service agreements and care plans

Required:

+ MN LPN licensure

+ AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

Preferred:

+ Able to work independently

+ Good communication and interpersonal skills

+ Previous healthcare experience

+ Comfortable using computers, hand held devices, phones and fax machine
