    Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.

     

    But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

     

    Street Medicine Coordinator

     

    Reporting to the Assistant Program Director, the Street Medicine Coordinator liaises with and supports the activities of the Street to Home Team’s medical providers (including psychiatric providers and Street Medicine Team (SMT). The Coordinator works closely with Housing Advocates and Case Managers to implement the recommendations from medical and psychiatric providers ensuring high quality care for clients.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    + Manage Street Medicine Team (SMT) schedule

    + Work with Housing Advocates and Case Managers to follow up on recommendations from psychiatry and SMT

    + Coordinate care with other providers (hospitals, clinics, specialists)

    + Communicate with hospitals and emergency rooms during client acute treatment/hospitalization

    + Communicate with SMT providers on the days providers are not with teams about emergencies, need for advocacy, other urgent matters

    + Develop relationships with providers in the community to facilitate referrals and care coordination

    + Gather medical information received from appointments or hospital visits

    + Find providers (primary care, specialty, additional health care services) for clients based on provider recommendations, geography, insurance coverage) and assist with insurance authorization process

    + Escort clients to medical appointments, labs and diagnostic imaging centers, as needed

    + Track clients’ Medicaid/Medicare certification to ensure recertification and prevent lapses in coverage

    + Assist SMT with coordination of special projects and public health initiatives such as vaccinations

    + Assist Housing Advocates and Case Managers in obtaining medical records from hospitals and other providers

    + Perform other duties as assigned

    MINIMUM QUALIFICATIONS:

    + Bachelor’s degree required; MSW or MPH preferred

    + Minimum 2 years of experience with the street homeless population as well as experience working in a Harm Reduction centered program

    + Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; f amiliarity with Access-based databases and the ability to learn new programs

    + Bilingual Spanish/English preferred

    + Valid NY, NJ, CT, or PA driver’s license with a good driving record, as defined by Breaking Ground is preferred
