    Responsibilities

    Mayo Clinic in Jacksonville, Florida is seeking a board-certified or board-eligible **Internal Medicine physician** to join a dynamic, multidisciplinary team caring for patients undergoing solid organ transplantation. This unique role is embedded within our transplant program and focuses on the longitudinal medical management of transplant candidates and recipients. The successful candidate will be integral to our developing Transplant Survivorship Clinic, providing comprehensive general medical care that spans pre- and post-transplant phases.

     

    **Responsibilities** :

    + Provide internal medicine expertise for patients undergoing evaluation for heart, liver, lung, and kidney transplantation.

    + Optimize management of chronic medical conditions including diabetes, hypertension, hyperlipidemia, and obesity.

    + Coordinate and deliver preventative health care, health maintenance, and cancer screening.

    + Collaborate closely with transplant physicians, surgeons, coordinators, and other specialists.

    + Support continuity of care across the transplant journey.

    + Contribute to the development and implementation of clinical protocols and quality initiatives in transplant survivorship.

    + Participate in educational efforts, including teaching internal medicine residents or fellows.

    + Engage in scholarly or quality improvement projects aligned with transplant care and survivorship.

    Qualifications

    + MD or DO degree, or equivalent, from an accredited institution

    + Board-certified or board-eligible in Internal Medicine.

    + Eligibility for medical licensure in Florida

    + Strong interest in interdisciplinary care.

    + Experience in complex chronic disease management preferred.

    + Excellent communication, collaboration, and organizational skills
