This position is in-person, primarily located at our Toledo Hospital, with the chance of floating to Toledo and Bay Park Hospitals.

POSITION SUMMARY

The Virtual Nurse assists with the coordination and management of care for patients at ProMedica Acute Care Hospitals via telehealth, which includes the use of telephone and virtual visit technology.

ACCOUNTABILITIES

Works collaboratively with other team members providing direct patient care, with prioritization of the following tasks or events done with the bedside team/PCS as follows:

Completion of admission assessment questionnaire on applicable/assigned patient’s

Participate in daily Daily Transitions Rounds Meeting.

Facilitate discharge process providing education to patient, family or primary care giver regarding medications, follow up provider appts needed, printing the AVS

Updates & individualizes care plans as applicable/assigned.

Focused on patient/family/care giver satisfaction, bedside nurse/team satisfaction, provider satisfaction, and patient safety

Functions as clinical resource to bedside staff & mentor to less experienced staff.

Other duties as assigned.

REQUIRED QUALIFICATIONS

Education: RN

Years of Experience: 3-5 years acute nursing

License: Ohio & Michigan

PREFERRED QUALIFICATIONS

Education: BSN

Skills: previous telehealth experience

Years of Experience: 1-3 years Virtual Nursing Experience

License: Ohio & Michigan
