    Salary: $2800 / Week

     

    The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

    Details of the position include:

    + Location: Framingham & Surrounding Areas

    + Duration: Open to contract, perm, or temp to perm

    + Setting: Home Care

    + Schedule: Days, can accommodate full time or part time (no weekends or holidays)

    + Salary: $70/hour plus mileage reimbursement

    Essential job functions and responsibilities:

    + Provides physical therapy services to patients according to a written physician’s plan of care period

    + Assessing and evaluating therapeutic rehabilitative slash functional status and participation into the development of a total plan of care period evaluates home environment.

    + Assessing for muscle strength, mobility gate, ROM potential for rehab.

    + Directing physical therapy treatment.

    + Instructing patients and families/caregivers in the use of in care of therapeutic appliances

    + determining priority needs physical therapy.

    + Reporting to position patients’ reaction to treatment or change in condition.

    + Initiating physical therapy program and instructs other personal and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of their physical therapy program for the patient by participating in case conferences.

    + Train patient in use of prosthetic devices.

    + Identifies the patient and family/caregiver needs for other home health services and refers as needed.

    + Prepares and submits clinical progress summaries based on the attainment of goals.

    + Participates in discharge planning for patient.

    + Prepares and submits a clinical process summary based on the attainment of goals as directed by the organization policy.

    + Consult to home families/caregiver indicated.

    + Education programs for nursing organization personnel as needed.

    + Participates in peer consultation process.

    + Supervisors and physical therapy assistance according to organization policy and state regulations.

    + When therapy is the only skilled service, instructs, supervisors and evaluates home health aide care every two weeks.

    + Performs other duties as assigned.

    Position Qualifications:

    + Possesses a degree from a bachelor's or master's program in physical therapy approved by accredited organization.

    + Licensed to practice as a physical therapist with through the state, if applicable.

    + Two years of appropriate experience as a physical therapist. Community/home health experience preferred.

    + Demonstrates good verbal and written communication and organization skills.

    + Possesses and maintains current CPR certification.

    + Must be a licensed driver with an automobile that ensured in accordance with state/or organization requirements and is in good working order
