Hospice RN Case Manager

**Location:** Dewitt to Mt. Pleasant, MI

**Schedule:** Monday – Friday, 8:00 AM – 5:00 PM

Your Role & Impact

As a **Hospice RN Case Manager** , you will bring comfort, guidance, and care to patients and their families during life’s most sensitive moments. You will:

+ Provide physical, emotional, and environmental assessments and develop individualized care plans

+ Deliver compassionate, hands-on nursing care in patients' homes

+ Collaborate with physicians and the interdisciplinary care team to ensure coordinated care

+ Support and educate families, helping them navigate the hospice journey

+ Respond promptly to changes in condition and adjust care as needed

+ Follow all care plans and medical directives with a focus on dignity and quality of life

What You’ll Need

+ Graduation from an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s)

+ Current RN license in the State of Michigan

+ At least 1 year of clinical nursing experience

+ **1 year of Hospice or Home Health experience is required**

+ Reliable transportation and willingness to travel for home visits

+ Ability to lift up to 50 lbs and perform standard physical duties of a nursing role

+ Strong documentation, time management, and multitasking skills
