Hospice Admission RN – Job Summary

$10k Sign on Bonus

The Hospice Admission Registered Nurse (RN) provides comprehensive nursing assessment, care planning, and coordination to support the comfort and well-being of hospice patients and families. This role ensures care aligns with the interdisciplinary plan and regulatory standards, while also managing the admission process.

Key Responsibilities

+ Conducts holistic patient/family assessments (physical, emotional, spiritual).

+ Develops and implements the hospice plan of care in collaboration with the care team.

+ Documents all care activities promptly and accurately; syncs documentation after each visit.

+ Communicates with physicians, hospice staff, and other providers to coordinate care.

+ Participates in admissions, on-call rotation, and interdisciplinary team meetings.

+ Educates patients and caregivers on disease process, care techniques, and available support.

+ Supports team operations, including visits to other hospice patients as needed.

+ Adheres to agency policies, quality standards, and compliance protocols.

Qualifications

+ RN license in the state of practice (1 year of RN experience required; CA requires 1 year by law).

+ Nursing degree from an accredited program.

+ Preferred: hospice or home health experience.

+ Excellent communication and interpersonal skills; skilled in grief and loss support.

+ Valid driver’s license and reliable transportation.

+ Ability to lift 35 lbs and travel to various care sites.

Supervision

+ Provides functional oversight to LPNs and Hospice Aides.
