The Clinical Intake Coordinator lays the foundation for a smooth operational process to give our participants the greatest experience possible.
Under the direction of the Home Health Administrator and Home Health Manager, the WelbeHealth Clinical Intake Coordinator maintains office functions of the home health department. The role focuses on answering phone calls, obtaining authorizations, scheduling the home health department, and maintaining medical records.
Essential Job Duties:
+ Assist in coordinating home health services for participants, home health orders from providers, and intake process with all involved parties
+ Enter patient demographics into any of the required electronic medical record (EMR) systems and provide clerical assistance to the Home Health Manager
+ Enter new participant data in EMR records and audit new admission paperwork to ensure medical records are completed
+ Ensure the appropriate notification of participants regarding their financial obligation, benefit coverage, and payor authorization for services to be provided
+ Work collaboratively with people and community referral sources seeking services
Job Requirements:
+ Graduate of an Accredited School of Nursing required
+ Unencumbered LVN License required
+ Minimum of two (2) years of experience with patient intake or medical office administration experience preferred
+ Demonstrated experience with Microsoft Office and data entry

Salary/Wage base range for this role is $66,560 - $81,395.73 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
