Our client, a leading organization in the medical and home care industry, is seeking a Client Care Coordinator to join their team in Oceanside, CA. This role is instrumental in ensuring that each client receives personalized and timely service, making their care journey as smooth and supportive as possible.

Key Responsibilities:

+ Gather detailed client information to develop and maintain accurate and individualized care plans.

+ Input and manage client data in the software system, ensuring accuracy and consistency.

+ Serve as the primary point of contact for clients and their families, addressing questions and setting expectations about care plans and services.

+ Coordinate caregiver schedules and duties to ensure client needs are met.

+ Organize and lead caregiver orientation and training sessions, ensuring caregivers are prepared to deliver exceptional service.

+ Promote professionalism, client-specific protocols, and effective communication in all caregiver interactions.

+ Act as the caregiver liaison, fostering positive relationships between caregivers and the organization to ensure effective teamwork.

Requirements

+ 2+ years of experience in home care, healthcare coordination, or a related field.

+ Exceptional communication, organization, and multitasking abilities.

+ A professional demeanor, particularly in high-end residential or client-focused environments.

+ Proficiency in healthcare management software is a plus.
