Unity House of Troy, Inc., a non-profit multi services agency dedicated to enhancing the lives of people in need has an opening for a Social Determinants of Health Liaison in our Supported Housing program for those living with HIV/AIDS.

This position has the option of a hybrid schedule.

Responsibilities:

Refer and link clients to transportation services that improve medical outcomes.

Assist participants to access emergency financial assistance, transitional housing and other resources.

Complete Social Determinants of Health assessments and other required program documentation.

Coordinate services with other providers in the community.

Provide referrals and linkages to care.

Obtain, enter and track data for reports related to funding compliance.

Promote a culture of inclusion and belonging.

Practice a harm-reduction service philosophy.

Requirements:

Associate’s or Bachelor’s Degree in health, human, or educational services or equivalent preferred. Experience may be substituted for degree. At least one-year experience working with those living with HIV/AIDS or other at-risk populations. Experience working with adults living with mental illness and/or substance abuse issues a plus! High School diploma is required. You must have a Valid and clean New York State Driver License and access to a reliable vehicle. Strong interpersonal, organizational skills, and experience/comfortability with computer systems and data entry required.
