    Position: High Impact Prevention Program Manager

     

    Hours: 37.5 /52 wks.

     

    Salary Range: $44,959 – $61,369

     

    Salary Grade: 8

    Summary of Responsibilities:

    Under the direction of the Action Front Center Director, the High Impact Prevention (HIP) Program Manager works in a team setting, planning coordinating, and delivering evidence-based HIV prevention, interventions, and direct services to individuals from targeted high-risk populations; and those living with HIV. This position is primarily responsible for supervision, management and coordination of services and assigned staff/volunteers provided through High Impact Prevention for Communities of Color initiative.

     

    The High Impact Prevention (HIP) Program Manager leads a team in strategic recruitment and community outreach efforts, aimed at engaging groups of high-risk men, women, and young women of color identified as at-risk/high risk populations; individuals living with/and or impacted by HIV/HCV/STI; and those identified as being most vulnerable. This position as program administrator, oversees the Agency’s commitment to policy and procedural development; advocacy priorities; research and special projects; and community efforts impacting communities of color. These efforts address obstacles/barriers to health/wellness, self-sufficiency and stability for health care, economic, mental health, social issues, etc. The position is responsible for overseeing and facilitating individual and group prevention interventions, client recruitment strategies, referrals to PrEP/PEP and treatment services, referrals to address social determinants of health, and linkages to other services.

     

    Services under the coordination of the HIP Program Manager are client centered; and include behavioral risk assessments; conducting and initiating linkages with health/medical services, partner providers, delivery of support services and follow-up to assure provision of treatment.

     

    This position is responsible for overseeing and conducting HIV/HCV/STI testing services at ABC, including maintaining ABC’s testing lab to optimal standards, maintaining testing supplies, staff training, policies and procedures, bloodborne pathogen exposure control, pre and post counseling, confirmatory testing procedures, documentation and reporting, and linkage and follow-up to treatment, PrEP, other STI screening, partner services, and other health care, prevention and support services. Additional duties include program/service development; planning & management; funding & resource development; fiscal management; staff recruitment, performance management and training; data collection, analysis, interpretation, and reporting; community & customer relations, and monitoring of services to assure compliance also assists in managing and monitoring the funding allocated through the AIDS Institute and applying for grant renewals and other funding sources that benefit the program.

     

    The High Impact Prevention (HIP) Program Manager participates in community collaborations and coalitions to promote health and wellness; and reduce health disparities. This position works to maintaining strong relationships with the NYS/County Department of Health/Contract Managers; and works closely with the providers/entities to promote the vision of the Agency/partners; and ensure program/consumer goals are successfully met.

    Qualifications and Education and Experience:

    Education: Bachelor’s Degree required, Master’s Degree preferred in Public Administration, Health Science/Public Health, Social Work, Nursing, Counseling, Organizational Management or related discipline. The equivalent combination of education and experience will be considered.

     

    Experience: Three (3) years of professional experience required working in health care/community education, public health, human service field, or comparable setting, preferably in a non-profit community-based program. Experience should include program planning/development; service integration; developing program goals/ work plans; compliance/program monitoring & evaluation; resource & fund development; performance management; training & leadership development; health education; and advocacy. Requires the ability to work independently with limited supervision; and the ability to use independent judgment, while also functioning as part of a team. Requires a minimum of three (3) years of supervisory & management work experience, or in a leadership capacity. Volunteer recruitment and management experience is a plus.

     

    + Requires excellent organizational and planning skills; good record keeping skills; and attentiveness to details in coordinating/monitoring program activities. Must have strong administrative, analytical, negotiations, follow-up, and problem-solving skills. Requires good time management skills; the ability to manage multiple priorities; and meet established deadlines. Should be flexible and able to work in a fast-paced, ever-changing environment; and the ability to work occasional evening and weekend hours, as needed.

    + Requires demonstrated success in the areas of program development, monitoring, reporting, and evaluation methods. Must have knowledge and experience in overseeing quality assurance, quality control and/or management measures of assigned services/staff/data systems. Requires experience evaluating key performance metrics and key result areas; interpreting program outcome measurements; and reporting/analyzing data. Knowledge of and experience in implementation of best practices in service delivery, assessment of service models, and service integration methods, is strongly preferred.

    + Must possess an extensive knowledge of HIV, viral hepatitis (HCV) and STI, prevention/intervention strategies and counseling services. Requires a strong knowledge of, and direct experience working with priority populations at high–risk and/or living with HIV or HCV. Must have knowledge of established guidelines for HIV testing and STI and HCV screening services, confidentiality/HIPAA standards, and other issues that can impact service delivery in both non-clinical and clinical settings.

    + Requires demonstrated experience in coordinating linkages to health, prevention, and support services, benefits, etc.; and in identifying beneficially related collaborative opportunities. Experience conducting individual screening assessments, developing service plans, goal setting, and active referral practices is required.

    + Requires the ability to assess individual and community needs; and develop service/advocacy recommendations. Strongly prefer familiarity with state/local service/health providers, advocacy efforts, research groups, committees, and public entities, etc.; and dedicated others who offer supportive services, opportunities, and relative information (i.e., County Health Dept, health and mental care providers, substance use treatment programs, etc.).

    + Experience working on fund/resource development initiatives and strategies is required. Must be familiar with the grant submission process; have the ability to identify and research funding opportunities; and assist in developing grant proposals for additional funding/program resources.

    + Must have experience in developing, coordinating, and implementing special events, meetings, forums, and supplemental activities. Experience in developing and disseminating informational, marketing/promotional materials is strongly preferred. Knowledge of public/media relations, marketing and social media is preferred.

    + Must have excellent communication skills, both verbal and writing; and must have excellent interpersonal and consumer service skills. Requires strong public speaking, presentation, and facilitation skills, knowledge of group dynamics, consumer and/or community engagement and team building strategies, are required.

    + Requires a high degree of confidentiality; must have a good understanding, comfort level, and ability to engage individuals for prevention/intervention, HIV testing and related topics.

    + Must have experience and ability to provide advocacy services on health/social.

    + Requires the demonstrated ability to interact with diverse individuals/groups at all organizational levels, in diverse settings; and in initiating beneficial partnerships/collaborative opportunities.

    + Requires the ability to maintain respect for cultural backgrounds and practices of employees/consumers; and to initiate and maintain partnerships with both internal & external consumers.

    + Requires proficiency in use of Microsoft Office products/suites including Word, Outlook, Power Point, etc.; and the ability to develop, secure, and maintain program data base systems, including AIRS.

    + Must be able to perform data input; maintain program/consumer information, etc. create reports, logs, etc.; and provide analysis and interpretation of data on consumer/program status in meeting outcomes; and develop related communications including announcements, procedures, presentations, information sheets, etc.

    + Requires the health and physical capability to work in an office and in a variety of community settings.

    + Bi-lingual capabilities in English and Spanish preferred.

    + Requires a valid NYS Driver’s License, and access to reliable transportation.
