Office Manager, Hospice

Responsible for overseeing the management of the administrative needs of the clinical services division. Reports to the Administrator.
Job Duties and Standards:
+ Manages the administrative needs of the clinical operations of the program, including, but not limited to, the following:
+ keeping track of patient data, i.e., referrals, admissions, deaths, etc.
+ entering patient data for the medical record;
+ entering patient data in Pharmacy Benefit Manager for all admission / terminations
+ oversees medical records – create new charts. Gather all initial information/documentation from RN and SW to process charts. Checklist form completed as information is received / print notes / assessments daily
+ oversees filing medical record forms in appropriate charts in a timely manner
+ print and fax monthly patient medication status lists to Pharmacy Benefit Manager and occasionally to physicians
+ review charts on a frequent basis to determine if records are in compliance with regulations.
+ completion of admission audits within specified timeframes
+ discussing with appropriate hospice personnel the admission data, i.e., essential information re: patient/family background, plan of care, and medical orders;
+ distributing referral information to all appropriate staff;
+ oversees keeping doctor’s orders, the plan of care, and certifications up to date;
+ oversees the delivery and return of medical records for M.D. signatures, in a timely manner;
+ seeing to the preparation of team meeting lists and maintaining team meeting notes;
+ ensure and prepare Interdisciplinary Team reports, and print and fax Care Plans after each Team Meeting to PCP;
+ monitor and provide certification due date notices for Team Meetings
+ divide / split charts as needed, maintaining updated clinical forms, booklets, and other clinical materials used by the staff. Assists billing staff as directed by the Administrator. Submits information to SHP for Family Surveys.
+ submits information to contracting vendors for Family Surveys;
+ maintains a calendar for Medical Director/Physician Designee and Palliative Care Provider, schedules face to face visits.
+ Assists with administrative duties as needed and assigned.
+ Performs other duties, as assigned, by the Administrator.
Qualifications:
+ + Education
+ Minimum Requirements: High School Graduate
+ Preferred at Hire: Same
+ Certification/Licensure (including annual requirements)
+ Experience (Years/Type)
+ Minimum Requirements: 2 – 5 years office experience Advanced proficiency w/ computer skills
+ Preferred at Hire: Same, Supervisory Skills
+ Equipment (Experience and/or Training)
+ Minimum Requirements: Phones, copiers, computers

+ Specialized Training
+ Minimum Requirements: Management skills
