We are seeking a talentedChildren’s Single Point of Access (SPOA) Facilitatorto join our team of nearly 500 employees. TheChildren’s Single Point of Access (SPOA)is centralized entry point to intensive behavioral health services and supports, both community-based and residential services. These services include but are not limited to; Health Homes Care Management, Child and Family Treatment & Support Services, Care Coordination, Skillbuilding, Youth Peer Mentor, Community Residence (CR), and Residential Treatment Facility (RTF). SPOA Facilitator works under general direction of SPOA Manager to create a seamless process from initial referral to linkage for youth and families who are eligible for these services.
CCSI is partnering with Monroe County Office of Mental Health MCOMH to hire aChildren’s Single Point of Access (SPOA)Facilitator. This position is employed by CCSI and working on behalf of MCOMH.
In this role, you will
+ Process incoming referrals, including inquiries from families and providers.
+ Provide general information regarding available services and referral processes to families, providers and the community. Be able to inform families of their rights and assist in understanding services.
+ Engage in outreach to youth, families and collateral contacts, as appropriate, to assist families in determining needs and makes referrals and linkages to appropriate programs.
+ Monitor, maintain, and prioritize the wait lists for SPOA services, including contact with families to provide ongoing support.
+ Obtain and track required information to process SPOA referrals from community providers, including completing a social history and trauma screening with families as needed.
+ Assist in determining service eligibility via comprehensive record reviews, family, referral source and provider input and supervisory consultation.
+ Assist in preparing and insuring timely submission of Residential Treatment Facility (RTF) referrals to the Pre-Admission Certification Committee (PACC).
+ Collaborate with and may provide consultation to SPOA programs to ensure Monroe County youth and families receive appropriate and effective services.
+ Enter and monitor tracking data in the database to manage SPOA services as well as Priority Youth/Children’s Psychiatric Center Diversion Youth and Children’s Psychiatric Center referrals.
+ Work in collaboration with Family Access and Connection Team (FACT) to ensure seamless referrals between service entry points.
+ Build effective linkages and relationships with informal and community supports, child and family serving systems, schools and other formal supports.
+ Lead and facilitate formal trainings on various topics related to mental health and suicide prevention.
+ Performs other duties as assigned.
Requirements
What You Bring
+ Bachelor’s Degree in mental health field, Master’s degree preferred, or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ).
+ Experience and interest in working with children and families.
Required for this position are:
+ Satisfactory completion of all required background screenings.
+ Able to work flexible hours including evenings
+ Valid Class D New York State Driver License and access to motor vehicle to meet transportation requirements of position.

Salary Description
