    Community Heart and Vascular has a part-time Ambulatory Triage LPN position available at the Cardiac Clinic. The Ambulatory Triage LPN is responsible for addressing patient's condition/symptoms virtually, giving appropriate clinical direction, education, and recommendation(s) for disposition.

    Exceptional Skills and Qualifications

    + Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.

    + Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA)

    + One to two years of ambulatory and/or triage experience in a clinical setting (Preferred)

    + Possesses knowledge of necessary computer systems including Microsoft Office, Outlook, and network electronic medical records (EMR).

    + Supports the mission, vision and PRIIDE values of Community Health Network.

    + Utilizes critical thinking skills, education, mentoring, guidance, and interventions to achieve goals of care virtually.
