Position Overview:

We are seeking a motivated and dependable Pharmacy Extern to join our Meds2Beds Program on a weekend-only basis. This role supports timely medication delivery and patient education for those being discharged from the hospital, playing a key role in reducing readmissions and improving patient outcomes.

Key Responsibilities:

+ Deliver prescribed medications directly to discharged patients.

+ Provide patient counseling on medication usage, potential side effects, and the importance of adherence.

+ Collaborate with pharmacists, nurses, and care teams to ensure smooth patient transitions.

+ Accurately document prescriptions and patient interactions in compliance with hospital and pharmacy protocols.

+ Provide coverage during weekends and holidays as needed.

Qualifications:

+ Enrolled in an accredited Pharmacy program.

+ Holds a valid pharmacy intern/extern license.

+ Current Basic Life Support (BLS) certification.

+ Strong interpersonal and communication skills.

+ Detail-oriented and committed to patient-centered care.

**Schedule** :

+ Weekends Only (Saturday and Sunday)

+ Occasional holiday coverage required
