    JOB SPECIFICS

    This is a lead professional position that provides work in the development, implementation and evaluation of HCBS waiver services, using a person-centered approach working to ensure equity, diversity and inclusivity. Duties include providing leadership, team development, direction and case consultation to case managers and assessors in the Access, Aging and Disability Division. The primary work area will be Consumer Directed Community Support services centering on training needs and the design of program implementation along with administering training and orientation to staff, service providers and people receiving services. There will also be focus on other areas such as oversight of out-of-home youth placements, community outreach and attending inter-agency meetings for collaboration on program implementation and policy oversight.

    DESIRABLE EXPERIENCE:

    + 3 or more years of paid work experience with Home and Community Based Services

    + Experience interacting with and providing services to individuals from a variety of cultural and ethnic backgrounds

    + Experience in working with individuals with disabilities

    + One or more years of experience in Home and Community Based Services (HCBS- waivers) is required

    + Demonstrated experience in understanding, implementation, and analysis of person-centered practice and diversity, equity, and inclusion work

    SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top scoring candidates will be forwarded to the hiring department for further consideration.

     

    HOURS/DAYS: 8:00 a.m.-4:30 p.m., Monday-Friday based out of the Stillwater Office. Work schedules (hours and days) and location are subject to change based on department needs. The position offers teleworking and flexible hours that align with the business need.

     

    ANTICIPATED HIRING RANGE: $77,188.80 - $105,352.00 Annually

    JOB SUMMARY

    The Social Worker, Senior is a lead professional position with significant responsibility for performing complex work in the development, implementation, provision, evaluation and fiscal management of services, programs, grants, and contracts.

     

    Client assignments of a Senior Social Worker may include more complex, sensitive or difficult cases than Social Workers l and ll and is also responsible for the consultation, training and support to Social Workers and professionals in and outside the agency.

     

    A Senior Social Worker may be assigned duties in any of the following units: Family/Children, Licensing, Chemical Health, Adult Mental Health, Development Disabilities, Adult Family Services, Minnesota Family Investment Program/WorkForce Center or Housing.

    DUTIES & RESPONSIBILITIES

    + Provide assessment and case management services for eligible individuals or families.

    + Assist clients in obtaining financial support (Social Security benefits, child support, housing, medical care and treatment for chemical or mental health needs).

    + Assess continued client eligibility for county provided services as needed.

    + Lead intensive consultations with staff and service providers for case planning, specialized programs and agency procedures.

    + Strive to keep clients at home through the use of other community resources and waivers funded by state and federal sources.

    + Explain legal processes and county systems to client families and community partners.

    + Maintain frequent and ongoing client contact either in-person or over the phone.

    + Provide leadership and guidance to staff team members.

    + Provide leadership, team development, direction and case consultation to unit staff.

    + Collaborate frequently with team members for consultation on team cases and for review of diagnostic assessments.
